You can accidentally get Cronobacter in powdered formula after you open the container. Cronobacter can live on surfaces in your home, such as a kitchen counter or sinks, and in water. In the home, Cronobacter could get into powdered formula in these ways:
If you place formula lids or scoops on contaminated surfaces and later touch the formula.
If you mix the formula with contaminated water or in a contaminated bottle.
In a processing facility:
Cronobacter can also get into powdered infant formula in a processing facility in these ways:
If the manufacturer uses contaminated ingredients to make the formula.
If the formula powder touches a contaminated surface.

Understanding Your Baby’s Risk
Cronobacter infections are rare, but they can be life-threatening in newborns. Infections in infants usually occur in the first days or weeks of life. About 2 to 4 cases are reported to CDC every year, but this figure may not reflect the true number of illnesses. That’s because most hospitals and laboratories are not required to report Cronobacter infections to health departments.
Cronobacter germs can cause a dangerous blood infection (sepsis). They can also make the linings surrounding the brain and spinal cord swell (meningitis).
Infants who are more likely to get sick include:
Infants younger than 2 months. These infants are most likely to develop meningitis if they get sick from Cronobacter.
Infants born prematurely.
Infants with weakened immune systems. Babies with this condition can’t fight germs as well because of illness or medical treatment, such as chemotherapy for cancer.
Cronobacter illness in infants usually starts with a fever and poor feeding, excessive crying, or very low energy. Some infants may also have seizures. You should take an infant with these symptoms to a medical provider as soon as possible.

Protect Your Baby From
Cronobacter
Here are steps you can take to help prevent your baby from getting sick with Cronobacter.
Breastfeed if you can. Very few cases of Cronobacter infections have been reported among infants fed only breast milk.
Clean, sanitize, and store feeding items safely. Germs can grow quickly if you add breast milk or formula to a partially used bottle or only rinse a used bottle, rather than clean it.
You can help keep your baby’s milk safe by carefully cleaning, sanitizing, and storing feeding items such as baby bottles, breast pump parts, nipples, caps, rings, valves, and other objects that may enter the baby’s mouth, like pacifiers and teethers. Take apart bottles and breast pump equipment after use so you can clean them thoroughly.
When cleaning surfaces, you may also use a disinfectant wipe or paper towel sprayed with cleaning product. Do not place feeding items directly in the sink, because germs in sinks or drains could contaminate these items.
Keep hands and surfaces clean! Before preparing and feeding bottles or food to your baby, it is important to clean surfaces, like countertops and sinks, with soap and water and to wash your hands with soap and water for at least 20 seconds. You should also wash your hands during these key times:
Before touching your baby’s mouth
Before touching pacifiers or other things that go into your baby’s mouth
After using the toilet or changing diapers
If soap and water are not available, use a hand sanitizer with at least 60% alcohol. Check the product label to be sure.
Wash hands with soap and water as soon as possible after using hand sanitizer. Hand sanitizer with at least 60% alcohol kills Cronobacter. But hand sanitizer does not kill all types of germs. It may not work as well if hands are visibly greasy or dirty.
Prepare and store your formula safely.
If you feed your baby with formula, consider using liquid formula when possible, especially if your baby is at higher risk. Powdered infant formula is not sterile and might have germs in it.
Liquid infant formula is made to be sterile (without germs). This means liquid formula should not make your baby sick with Cronobacter infection when you follow the instructions on the container.
You do not need to warm infant formula before feeding, but some people like to warm their baby’s bottle. If you do warm the bottle, never use a microwave. Microwaves heat milk and food unevenly, resulting in “hot spots” that can burn your baby’s mouth and throat.
Make sure your formula is not expired or recalled. The container should be in good condition with no dents, puffy ends, or rust spots.
To warm a bottle, place it under warm running water. Keep the running water from getting into the bottle or on the nipple. Put a couple drops of infant formula on the inside of your wrist to make sure it is not too hot.
If you feed your baby with powdered infant formula, it is even more important to keep surfaces, feeding items, and your hands clean to prevent germs that can make your baby sick from getting into the formula container.
Don’t set the formula scoop on the counter, in the sink, or on any other surface. Keep lids and scoops clean and completely dry, and close containers of formula as soon as possible after using them.
In most cases, it is safe to mix powdered infant formula following the manufacturer’s instructions on the container. But if your baby is at higher risk, consider taking these extra steps to prepare your powdered formula with hot water (at least 158°F/70°C):
Boil water and let it cool for about 5 minutes. It is important to mix the formula while the water is still very hot so that the heat can kill any germs.
Pour the water into a clean bottle or feeding cup.
Add the exact amount of formula listed on the container. Then, put a cap on the bottle and shake to mix. Don’t use a spoon or other utensil to stir because this could introduce germs.
Cool the formula to body temperature to ensure it is not too hot before feeding your baby. Hold the capped bottle under cool water or place it into an ice bath. Do not let the cooling water get into the bottle or on the nipple.
Test the formula’s temperature by putting a few drops on the inside of your wrist. It should feel warm, not hot.
Use prepared infant formula within 1 hour from start of feeding and within 2 hours of preparing it. Throw away any formula your baby does not finish. Don’t refrigerate leftover formula to save it for later. The combination of the formula and your baby’s saliva can allow germs to grow.
If you do not plan to start feeding your baby with the prepared formula right away, put it in the refrigerator immediately. Use formula in the refrigerator within 24 hours. Throw out formula if you can’t remember how long you have kept it in the refrigerator. Do not feed it to your baby.

Suctioning Your Child’s Nose and Mouth | Way to Grow
Your child may need to have his/her mouth and nose suctioned with a bulb syringe or with a suction catheter. Parents and all hospital caregivers can use a bulb syringe. We will teach you to use a bulb syringe before you go home. A nurse, doctor or respiratory therapist will suction the back of the nose and throat to reach mucus that is too far back in the throat to be removed with the bulb syringe or a plastic tipped suction catheter.
Why does my child need to be suctioned?
A stuffy nose can make it difficult for your child to breathe. This can make your child fussy, especially when he/she tries to eat or sleep. Suctioning is necessary when an illness causes the body to make too much mucus. Some examples of illnesses that cause the body to make excess mucus are:
A common cold
Respiratory Syncytial Virus (RSV)
Bronchiolitis
Pneumonia
Influenza
Suctioning is also necessary when cough reflexes are not strong enough to get rid of mucus, saliva or vomit.
Cough reflexes are not fully developed in some infants, especially preemies.
Illnesses or injuries that affect the nerves or the brain can affect the ability to cough. Excess mucus can accumulate in the back of the throat, nose and mouth.
Deep coughing to clear mucus can be difficult after some types of surgeries.
How to make salt water solution for bulb suctioning:
Mucus can be thick. A salt water solution can be used to help thin the mucus and make it easier to remove. A salt water solution (saline) can be bought in the drug store or you can make your own at home using the steps below. Use salt water solution up to 4 times a day for suctioning.
1. Mix 1/4 teaspoon table salt and 1 cup boiled water.
2. Allow to cool to room temperature.
3. Store in a clean, covered jar or bottle. Label with the date it was made.
4. Throw away after 3 days. If you need more, make a new mixture.
Suctioning with a bulb syringe
Your baby cannot blow his/her nose, so you need to use a bulb syringe to remove excess mucus. A bulb syringe (or suction bulb) is a small rubber object with a long tip at the end of a bulb.
Have a bowl of tap water ready to clean the bulb between suctioning attempts.
There are several ways to position your child so he/she does not push you away or wiggle out of your arms. Your child’s nurse will help you find the best position for your child.
If both the mouth and nose need to be suctioned, suction the mouth first. When suctioning the mouth, place the tip of the bulb syringe towards the inside of your child’s cheek.
Wash your hands before and after suctioning.
Hold the tip of the bulb between your middle finger and forefinger. The bulb should touch the palm of your hand. Before inserting the tip into your baby’s nose, use your thumb to push out the air. If the mucus is thick, put 2-3 drops of salt water solution in your child’s nose before inserting the bulb syringe in your child’s nose.
Insert the tip of the bulb into either the mouth or the nose and slowly release your thumb. Suction is created as your thumb releases pressure on the bulb. This will remove the mucus or fluid from your child’s nose or mouth.
If the bulb does not reinflate, this is usually caused by the tip being against the cheek or lining of the nose, or because the tip is blocked by thick mucus. If pulling back on the bulb does not reinflate the bulb, remove and clean the bulb syringe.
Remove the bulb syringe from your child’s mouth or nose. Use your thumb to push mucus or fluids out of the bulb syringe onto a tissue or paper towel.
Repeat as needed. Allow your child to recover and breathe between each suction attempt. Gently wipe your child’s nose with a tissue as needed.
When finished, clean the bulb syringe using a bowl of soapy water, pulling the soapy water into the bulb and squeezing it out. Let the bulb syringe air dry.

When should I suction my child’s nose or mouth?

Before feeding or nursing if his/her nose is stuffy. Your child will eat better if his/her nose is cleared. Suctioning too soon after eating or drinking may cause vomiting.
If your child is having difficulty breathing after he/she vomits or “spits up.” Try to limit suctioning to two to three times a day. Suctioning more often may cause the inside of the nose to dry out, get sore and bleed.

Nasopharyngeal (NP) suctioning
A nurse, doctor or respiratory therapist may decide to use NP suctioning if:
Mucus cannot be removed with a bulb syringe or rigid plastic-tipped catheter (Yankeur).
They hear abnormal breath sounds when they listen to your child’s chest.
Your child needs oxygen or more oxygen than usual.
Your child is working too hard to breathe, breathing too fast, or has difficulty breathing and eating at the same time.
A small suction tube is connected to a suction device. Salt water solution may be used to thin and loosen the mucus and to moisten the inside of the nose. The tube will be gently placed in your child’s nose until it touches the back of his/her throat. This makes most children cough. The coughing will help bring up the mucus to the back of throat where it can be removed. The tube will be gently and slowly pulled out of your child’s nose while suction is applied to get rid of the mucus. This may need to be done several times in each side of the nose. After your child’s nose is clear, another suction tube may be used to suction his/her mouth.
Are there any problems with NP suctioning?
The inside of your child’s nose may become swollen if he/she needs frequent NP suctioning. Also, your child may have a mild nosebleed. If this happens a smaller suction tube or a different tool called a “neosucker” can be used until the bleeding or swelling is gone.
If your child has to be suctioned within 30 minutes after eating, he/she may vomit.
NP suctioning is only done when it is necessary. Although suctioning may upset your child for a few minutes, he/she will feel better and breathe easier once the excess mucus is cleared.

In Russia every third child belongs to the risk group for health reasons. And the largest group is children with mental disabilities. According to UNESCO and WHO, for every 100 schoolchildren there are up to five people with intellectual disabilities, for every 800 newborns there is one baby with Down syndrome.

How can parents understand that their child is different? Why did something go wrong in the development of the baby at some point? Where and how to teach a student with a diagnosis of “mental retardation”? Is it true that even the common flu can lead to such a diagnosis? Tamara Isaeva, Associate Professor of the Department of Oligophrenopedagogy and Special Psychology of Moscow State Pedagogical University, answers the questions of “RG”.
Tamara Nikolaevna, are intellectual disabilities in a child an inborn thing?
Tamara Isaeva: It happens in different ways. Violations may appear at different times. This may be an intrauterine lesion or a lesion that appeared later, during the maturation of brain structures. For example, a child was born healthy, but suffered encephalitis or menningoencephalitis: the inflammation affected the higher parts of the brain, the youngest, and an irreversible process occurs.
Another situation: the child was born healthy, but has a hereditary predisposition. A variety of factors can lead to the appearance of mental retardation: for example, the common flu. Sometimes mothers say: everything was fine, suddenly – a sharp rise in temperature and a sharp rollback in development. This is most likely the hereditary factor. We usually talk about mental retardation when exposed to adverse factors under the age of 3 years.
What signs should alert the parents of a baby?
Tamara Isaeva: Here are five signs that will allow parents to understand whether their child is at risk of developing certain developmental disorders.
1. The baby does not look for contact with his mother with his eyes, does not respond to his name, he does not have a response to the voice.
2. Child does not sit at six months (with support), did not start walking at one year.
3. A child from 1 to 1.5 years old is not interested in the objective world, toys, does not try to imitate.
4. The child does not have a pointing gesture (he does not point his finger at the toy he wants to receive, at the bottle of juice or water).
5. By the age of three, the child does not begin to say “I”.
And what if the child does not speak by the age of 3-4?
Tamara Isaeva: The first thing parents usually notice is that the child does not speak.
I had a case: a child is 7 years old, and he does not eat on his own and does not get dressed. What is more important: talking or dressing yourself? Mom only cares about the speech, “I’ll dress him myself.” But when we block the skills of dressing and undressing, then we, consequently, block the development of speech, especially its communicative side. Speech develops and becomes not by itself, but in conjunction with many skills. As a child learns to dress and develop, very subtle differentiated hand movements develop. And this is the development of the articulatory apparatus and speech. Therefore, here it is necessary to think about what comes first and what comes after. That child had mental retardation.

By the way, in the West they do not make such a diagnosis at all, these children study in ordinary classes. As a rule, their pace suffers, they are slower compared to other students. But if they create favorable conditions, the result will be no worse. Often this is not taken into account, including when passing the exam. For some reason, it is believed that all children work at the same pace. This is wrong.
Where to teach a child with intellectual disabilities – in an inclusive school, a correctional school, or are there children who are not taught at all?
Tamara Isayeva: There are no uneducated children. Education is, first of all, the development of a person. If a child does not know fractions or the multiplication table, this does not mean that he does not develop. But everyone has their own capabilities, abilities, and on this basis, it is necessary to train such children. In the process of learning, everyone shows abilities. For children with intellectual disabilities, the educational environment is very important. They have very few internal resources, it all depends on how competently, seriously, patiently the defectologist works, who follows the path of restoring disturbed ontogenesis. This effect favorably affects the formation of neural connections in the cerebral cortex.
I can rely on my own pedagogical experience, when my students, after graduation, did not know how to speak, read and did not master writing, like you and me, but at the same time they were socially adapted. And for such people, the most important thing is to be happy in the simplest life.

It turns out that not every child can be taught to read and write?
Tamara Isaeva: Someone writes and reads the same as we do, but there is also the so-called alternative writing, global reading methods. We use the analytic-synthetic method in mastering writing and reading. And there are children who read in whole words. Of course, they have a small vocabulary, but they have it. There is reading at the level of pictograms, symbols. After all, we meet, for example, instructions for new technology, where there is not a word in Russian, but there are pictograms, and we understand everything. So are these children.
Special school students used to have to graduate at 18, no matter what they had learned by that time. Now in schools they can be up to 23 years old. What’s next?
Tamara Isaeva: If we take a mild degree of mental retardation, then there is infrastructure for such people, the opportunity to find a job. But if the children are heavier, then everything is much sadder. So far, there is only one way for them – to psycho-neurological boarding schools. I once worked for eight years in such a boarding school and I know that many of the patients could live in society.
Do they need to work?
Tamara Isaeva: Yes, but if it is not supported, then such a need fades.
In maternity hospitals, mothers are still told: leave a sick child, he will not even recognize you later. This is true?
Tamara Isayeva: It’s not true! A child has an innate need to be in communication with his mother or with a person who, from birth, began to take care of him.
Do your students remember you? Do they recognize you, can they write you a letter?
Tamara Isayeva: Not all. Someone recognizes on the street, smiles, someone does not. It doesn’t upset me. Another thing is upsetting: our society of high achievements does not allow people with achievements, at the level of their capabilities, to live in this society. And that’s the problem.

Many mothers complain that their children suck their fingers or the edges of clothes, bite their nails and toys, pick at their navels, wrap their hair around their fingers, etc.
“And what did I not do with it! – mothers complain, – Well, she explained in a good way, and scolded, and punished, and promised to buy a toy! I can’t get him out of this stupid habit!”
Indeed, no “educational measures” will not help and even aggravate the situation until the true causes of such behavior are clarified and eliminated. If your child commits certain obsessive actions, this is usually the result of some kind of psychological or physiological discomfort. Let’s find out together with you the possible causes of “bad” habits.
The first group of reasons
If a “harmful” habit went smoothly from the infantry sucking of the fingers or arose shortly after parting with the dummy, then, most likely, its reason lies in the problems that arose during infancy.
Contact with mother

Sucking reflex

Some babies have a stronger sucking reflex, some have a weaker one. Therefore, one newborn is enough for those 30-40 minutes that they spend at the mother’s breast during feeding, while others feel the need to suck something in between feedings: a pacifier, a fist or a corner of a diaper.
It is desirable that the baby spends as much time at the mother’s breast as he needs not only to get enough, but also to satisfy his sucking reflex and get a sufficient “portion” of physical and emotional communication with his mother.
Usually, if the baby has satisfied all of the above needs, he, satisfied, releases the nipple on his own or falls deeply asleep at the mother’s breast.
It happens that, for one reason or another, breastfeeding is not easy. This is where a certified lactation consultant can help. But if you decide to stop breastfeeding and switch to formula, then there is a danger that the baby will not be able to suck, he will not be able to work out his sucking reflex. The fact is that the milk formula flows quite freely from the bottle, while while sucking the breast, the baby has to make significant physical efforts in order to “get his own food”.
Formula-fed babies usually need extra suckling, so they should be able to suck on a pacifier in addition to feeding, and as much as they need.
Often, when such a child grows up, he still likes to drink from a bottle with a nipple or suck on a pacifier for quite some time.
In such cases, some parents become nervous and try to wean him from both. But if the child has not yet worked out his sucking reflex, he, having lost a bottle or a dummy, is forced to suck his finger, and this takes root in him as a habit.
B. Artificial feeding makes it difficult to satisfy the sucking reflex in infants. An undeveloped sucking reflex can develop into the habit of sucking fingers in formula-fed babies .
Therefore, such children should drink from a bottle with a pacifier or suck on a pacifier for as long as they need to. And it must be taken into account that a child who suckles at the breast develops his sucking reflex much earlier than an artificial child, who may need to suck even at 3-4 years.
In addition, the problem of an unsatisfied sucking reflex can also occur in a child who was breastfed. Some mothers strive to wean the baby from the breast as early as possible, for example, at 7-8 months, citing the fact that the baby is already big and can drink from a cup or cup.
However, it has been observed that when a mother breastfeeds her child for more than a year, as a rule, the child does not develop the habit of thumb sucking or anything else. So, do not worry if your one and a half year old baby is tied to your chest. That’s the way it should be.

Teething period
When the first teeth begin to come in, the child may become irritable and moody from time to time.
This happens not because his character suddenly deteriorated, but because his gums itch and itch during this period. The sucking reflex, which, perhaps, had already begun to weaken by this time, again becomes very strong, and the baby is ready to suck the mother’s breast or nipple to exhaustion even when he is full, and sometimes not only suck, but also bite hard.
During this period, care must be taken to ensure that the baby can freely satisfy his need to chew and suck something. Safe rubber toys, a rubber ring, teethers should always be within his reach.
D. The habit of sucking fingers is sometimes formed during the period of teething if the child did not have the opportunity during this period to gnaw and suck something.
Oral stage of knowledge of the world
From about three to nine months, there comes a period when babies put everything in their mouths. At this time, the child actively learns the world around him with the help of all five senses, and taste buds play an important role here. During this period, it is necessary to provide the baby with the necessary and reasonable freedom in the study of subjects.
He must be able to touch objects, pick them up, knock, put them in his mouth, in a word, explore all their properties.
To do this, safe toys suitable for such exploration must be available within the child’s reach. Also help your baby explore the world outside the area where he usually plays.
Try to keep things that your baby touches more or less clean and safe so that you do not have to pull the baby every time: “Ugh, what a bug! You can’t put it in your mouth!”
If during this period the child grows in a calm environment, his mother loves him, understands him, pays enough attention and warmth, provides reasonable freedom in studying objects through taste buds, fully develops, tactfully switching his attention from unwanted actions, then the baby does not have to look for compensation and calming in external stimuli.
E. If the child has not had the opportunity to safely survive and complete the “oral” (or oral) stage, then this unmet need may develop into compulsive thumb sucking or nail biting (etc.).
What to do if your child already has the above “bad” habits?
Try, if possible, to fill the child’s missed needs. To do this, give the child as much time as possible, caress him, kiss, show your love.
During the day, massage the area around the mouth, do a general massage, while paying special attention to the limbs: feet and hands.
The child’s mouth can be occupied by “nibbling” carrots, apples, crackers. And hands – finger gymnastics and games for the development of fine motor skills.

SECOND GROUP OF CAUSES
It happens that a “bad” habit occurs “out of the blue” in children older than 2 years. Then the cause of its occurrence, most likely, is not associated with the sucking reflex, but is caused by prolonged or severe stress. In this case, involuntary obsessive actions serve as a way for the child to calm down and are caused by self-regulation of the nervous system.

An unfavorable environment for a child is not necessarily something egregious, like, for example, always rowdy alcoholic parents. Even outwardly quite prosperous atmosphere can be very unfavorable for the child. For example, strained and cold relationships between adults, lack of mutual understanding between them, as well as inconsistency and lack of a single line of education can negatively affect a child and serve as a source of long-term stress. An equally heavy burden for the child is increased anxiety or nervousness, excessive irritability or irascibility of the parents.
In addition, if the parents are too busy with their own problems and troubles, the child feels lonely, he is too left to himself.
The reverse situation also happens. Parents are overly concerned with raising their child. They make excessive demands on him, make him attend a huge number of circles and expect dizzying results, hoping that their crumbs will leave their peers far behind in their development.
Such chronic overwork and fear of not meeting parental expectations can be overwhelming for the child and also become a source of stress for him.
Due to any of the above negative aspects, the baby begins to feel discomfort, nervous tension or fear, feels insecure.
What to do?
Try to establish a trusting relationship with your child, be attentive to him, find out his anxieties and fears. Help overcome them, soothe, distract and do not forget to cheer.
Hug and kiss the baby more often, rejoice together in his achievements and, together, grieve over his failures. Sincere empathy is very important for your child!
Teach him to worry not only about himself, but also about others (transfer his attention to other people).
Be consistent in established rules, prohibitions and requirements. Remember that there should be few real prohibitions.
Reduce the emotional and intellectual load of the child if his stress was associated with overwork. Or, on the contrary, increase the emotional and intellectual content in his life, if you understand that before that your child was alone, left to himself and he lacked external impressions.

What to do? Regularity and stability of his daily life is very important for such a child. I would like to add to the above that, of course, you can’t escalate the situation, focus the child’s attention on his problem, scold, blackmail, punish, forcibly remove your fingers from your mouth, smear them with mustard, etc.
It is better, as if by chance, to distract the baby with the help of toys, books, errands, and, simply, hugs or kisses.
It is impossible to expect that your actions to wean a child from a “bad” habit will bring quick results.
Most likely, even when the stress is removed or significantly compensated, the baby will bite his nails, suck his thumb or perform other obsessive actions for a long time. Over time, under favorable circumstances and adequate parental support, when the child’s nervous system gets stronger, this habit will disappear by itself.

When you break it down, the variance in pay according to location is due to a handful of reasons. First, the cost of living depends on where you live. Population—and therefore the concentration of people who need medical care—can also impact how much healthcare organizations are able to pay.
The West Coast (including Hawaii and Alaska) offers the highest median salaries for medical records and health information specialists according to BLS data, which includes billers and coders. Even though the responsibilities and day-to-day work is similar for billers and coders no matter where they work, the type of work setting influences how much you are paid. After all, as a biller or coder, you may be qualified to work in all sorts of healthcare facilities. (Remember, you’ll need to tailor your education depending on where you want to work, since coding systems vary among healthcare settings.)
Insurance agencies tend to pay billers and coders the most. Medical and surgical hospitals salaries are around the middle of the pack. Median salaries are lowest in physician’s offices and outpatient care centers, according to the BLS.
Not surprisingly, insurance agencies tend to pay billers and coders the most.
Keep in mind that some medical billers and coders work from home. These positions can either be remote jobs for employers such as a hospital or clinic, working for an independent company that serves healthcare facilities, or as an independent contractor who works directly with healthcare organizations. Pay in these work-from-home environments may not directly match overall trends.
Before you make the leap to working from home, investigate how much others make doing the same thing in your area.
Your Certifications
Earning certifications and credentials influence your pay in several ways. First of all, they provide evidence of your skills, potentially making you an attractive candidate. “If you have coding credentials, that looks good on your resume,” explains Robyn Korn, MBA, RHIA, CPHQ, an adjunct instructor of medical coding at Purdue University Global. “They show you have a knowledge base employers are looking for, making them more likely to consider you.”
Secondly, certifications and credentials may be able to help move you up the pay scale, no matter the salary you started at.
Some credentials are very specific and demonstrate your ability to fill niche positions. For example, certifications in ophthalmology or gastroenterology are incredibly specialized, and not many coders earn these credentials. That usually translates into higher pay for positions that require this niche training and education.
Your Pay Structure
When you’re hired as a medical biller, medical coder, or both jobs, you may be paid as an hourly (non-exempt) employee or salaried (exempt) employee. When you work hourly, you will need to clock in and out to accurately log your hours.
Overall, coders and billers who work more than 40 hours a week are paid more—no surprise there. Some of this increase comes from hourly workers who earn overtime, which is often 1.5 times the hourly rate.
Employers prefer to avoid paying overtime, though, so this scenario is more likely to play out when an organization is struggling to hire enough billers and coders to fulfill needs.
Some billers and coders with salaried positions work more than 40 hours. Workplaces that have the expectation that salaried employees will put in more than 40 hours a week may compensate accordingly by offering a higher salary.
Independent billers and coders, and those who work for outsourcing companies that serve healthcare organizations, may be paid differently. Other common pay structures include per-case or per-claim or a set percentage of the amount you claim.
How Can I Earn More?
You can’t control some things that impact your pay: For example, you can’t fast forward in time so you suddenly have more experience. That said, billers and coders can take steps within their power to earn more.
Most important is to be thoughtful and strategic about which steps you take—and in what direction.
Not long ago, several people on Korn’s staff wanted to advance their careers, so they started a coding program. The problem: The program didn’t focus on the type of skills needed to grow their career in the way they wanted. “A program or certification only takes you where you want to go if it aligns with your goals,” Korn explains.
Education:
More education generally translates to a higher income. Can you take classes part time to earn an associate or bachelor’s degree? You can increase your earning potential by studying online or in person.
Certification:
Medical coders who have a basic credential, such as a Certified Professional Coder (CPC), generally earn more than those who have finished a training program but haven’t sat for a certification exam. You can also “stack” certifications, adding more throughout your career to potentially increase your pay.
Networking:
“When you network with other professionals in the field, you open yourself up to advance your career,” Korn says.
Job Growth and Outlook
The career outlook for medical billing and coding is promising: Employment in this field is projected to grow at 6.6% through 2031, just above the national average of 5% for all jobs.
The BLS says an aging population is partly driving the demand for medical billers and coders. As patients age and live longer and with more chronic illnesses, health care organizations likewise need to grow to accommodate these changes. Facilities will need medical coders and billers to ensure they are properly reimbursed for this uptick in care.
Korn notes a trend of increasing use of electronic coding systems and auditing software. These tools are unlikely to fully replace professionals, though. An experienced biller and coder will likely be needed to manage the huge stream of electronic information, ensure the correct codes are used, and ensure an efficient flow of revenue.
Resources
The American Academy of Professional Coders (AAPC) offers ongoing education and workshops, professional networking, and certifications. “The AAPC focuses more on coding in physicians’ offices and clinics,” Korn says, so this is a particularly important resource if you’re interested in those settings.
The American Health Information Management Association (AHIMA) provides certifications, education, conferences, and advocacy. “If you’re not sure what setting appeals to you, you can rely on AHIMA because it gives you exposure to both physicians’ offices in addition to hospitals,” Korn says.

I am a mother on maternity leave. I went on maternity leave from my job as an administrator at the creative center, and found a remote job, also as an administrator at the children’s center – it was boring to sit at home.
I took a break for a week while I was in the hospital, then I continued to work and take care of my son – I have a super calm baby, so there was enough time.
Gradually, sitting within four walls all alone became completely unbearable – I live very far from my family, my friends moved away during the decree, plus I was very worried that I was not developing in any way, and I had enough money with a child to live only from paycheck to paycheck.
So I started looking for a second job – in a year and 9 months of the decree, I had three of them.
First, I got a job as an administrator in a company integrating a large CRM system. It was great with them, but I worked there for only three months – the SVO began and they massively reduced line staff.
Then I worked as a business assistant for an entrepreneur for almost a year – he is engaged in several projects: a restaurant, a semi-finished products production, a computer club; I also did several projects in the field of information business – I sold various online trainings.
At this job, I realized that you can sell people any nonsense if you sell well and your conscience does not bite you – I was also very annoyed by periodic supposedly urgent tasks on weekends that appeared only due to disorganization in project management. I spent some time organizing tasks, cases and ideas within projects, but it didn’t give much result – the leader had a rather chaotic style of work, so I just burdened myself even more with such putting things in order.
I quit my job myself because I was very tired. It was hard to cope, constant stress due to far-fetched deadlines, plus my one and a half year old son was with me all the time – children at that age need much more attention, so I couldn’t rest on weekends either.
I came to my senses for a couple of months – I continued to work as an administrator at a children’s school, but there was a small load and a very adequate leader, so I felt like I was on vacation.
After my dismissal, there was less money – my husband has an unstable income, so you can’t count on him. When the money ran out again three days before the salary, I wrote to an HR friend – she just had a job vacancy for which she recommended me.
I’m finishing my test week now. This is a fairly large company – they want to build a new direction for promoting the product, and I’m just testing options from which it will be possible to build this direction later. In my heart I hope that everything will go well and I will finally have a stable job with a good salary and an adequate level of stress – but I’m not thinking ahead yet.
Schedule
I work at the computer while the child is sleeping – this is in the morning from 4.30 to 6.30 and in the afternoon from 12 to 14. At this time, I try to do complex analytical tasks that require concentration.
The rest of the time I work from my phone — I communicate with colleagues and clients, write simple texts, make payments. Usually I sit on the floor, my son plays nearby; sometimes I turn on cartoons – then we lie on an armchair in an embrace, with my free hand I work through a laptop.
Sometimes I need to call for work – my colleagues know that I have a child, and they treat the sounds in the background with understanding. I put calls with clients on daytime sleep – they are rare.
Usually I can finish everything by 17 o’clock, if there are no force majeure – at the same time I take breaks for breakfast, lunch, snacks, laying down for a nap, changing a diaper and other urgent children’s affairs.
The rest of the time I try to spend as much as possible with my son, but I often miss active games — so I take him to the playground and watch him play in the sandbox.
Situation
At the end of December, I was on the verge of a nervous breakdown – I had no strength, my son was teething and he really needed his mother, and my supervisor kept throwing up very urgent tasks, although everyone around had already cut Olivier.
I worked myself up so much that it became physically difficult – my head ached a lot, constant nausea appeared. There was no talk of any concentrated work here, of course – I somehow managed to finish the most burning thing, and rake the rest after the holidays.
After this incident, I am anxious about the weekend – I turn on the “do not disturb” mode and do not read messages. If the situation is really important, they call me – everything unimportant endures until Monday.

Sleeping 8 hours every day is the foundation of good health. I stick to the regime no matter what and it really works.
Sports — I start workouts at home from time to time, but I don’t have enough for regular regular classes. In general, the mood rises, as if strength is added, you feel better emotionally – I plan to definitely introduce myself to the gym or dancing in the future.
The coolest life hack – you work better if your child doesn’t bother you every 30 seconds. At home, I did not work out with assistants, but all February I stayed with my son with my parents – and this had the best effect on work, health, and relations with the child.
Consequences
So far, the only consequences are burnout and exhausted nerves.
I want to save up a financial cushion, but my husband and I have a common budget, so all the savings go to general needs – it turns out work is zero.
On the plus side, my son goes to a paid pediatrician and eats varied meals. I acquire valuable skills for which I am paid more, and I will not leave the decree into the void.
Of the minuses – I miss how my son grows up. While I’m typing ad copy for someone else to make a lot of money, my toddler is turning into a little preschooler. The worst thing, it seems to me that he has come to terms with the lack of my attention, feels lonely and very sad.
I would not advise anyone to combine caring for a child with another load – of course, there are many different situations, and maybe your parents, husband or nanny help you. But if this is to the detriment of you and the baby, then it is better to take care of yourself now – wait for kindergarten and calmly go to work.

Hello! My name is Demetria Kitchens and I am honored to be the Center Director for The Sunshine House Early Learning Academy located in Stockbridge, Ga. I have worked in the field of Child Care for more than 15 years. I started my career as an After School teacher.
I am also a former Third Grade teacher. One of my biggest joys is being hands-on in the different classrooms at the Center. I love education and my belief is that education truly starts at infancy. There is no greater joy than seeing children grasp what is being introduced and taught to them and they are enjoying it. I love working with families and assisting them in any way possible. I would absolutely recommend this Sunshine House! My son has been here for over 5 years! The staff has little turn over so we have gotten to know them over the years. It is nice to know the people you are trusting with your child. They are always friendly and smiling! Now that he is school age they pick him up from school and make sure that he has time to work on his homework, which is very helpful. During the summer, the summer camp program is awesome! They arrange fun field trips and have great in house activities for non field trip days. I feel comfortable in knowing that my son is safe and happy when I am working!

My name is Brittany Deluca. I am married and have two sons, who have both attended The Sunshine House since they were infants.
I was born and raised in Charlotte. My family has lived in Mint Hill since 2008 and we have fallen in love with the community. I enjoy singing, dancing, creating, and spending quality time with my family and friends.
I have a Bachelor’s Degree in Education and have worked in the education field since 2006.

More than 75 million children aged three to 18 from countries torn by civil conflict, natural disasters and other crises are in desperate need of urgent assistance in the field of education. However, only two per cent of the funds provided annually for humanitarian purposes are allocated for these purposes.
The Children’s Fund published new statistics on the eve of the World Humanitarian Summit. It will be held May 23-24 in Istanbul. The summit is called upon to reaffirm the commitment of the international community to the principles of humanity, unity and cooperation, without relying on which it is impossible to prevent crises, to find ways out of them and save people from suffering.
The United Nations Children’s Fund (UNICEF) has pledged to raise $4 billion in educational assistance over the next five years for 13.6 million children in humanitarian emergencies.

As part of this campaign, Hollywood actor and UNICEF Goodwill Ambassador Orlando Bloom traveled to eastern Ukraine, where he visited schools and kindergartens whose buildings were damaged during the hostilities. The purpose of the trip is to remind about the importance of education for children affected by the conflict.
“Education is an important foundation for building a new life in a safe and supportive environment for these children,” said the UNICEF Goodwill Ambassador.
As a result of the conflict in eastern Ukraine, approximately 580,000 children were placed in need of assistance and more than 230,000 children were forced to leave their homes. Every fifth school in the conflict zones was destroyed. Approximately 300 thousand children need help in the field of education.
“I met children. Liana, 11, described how she hid in her school’s basement for almost two weeks without light or heat, and it was freezing outside. Sitting in the basement, the girl heard how artillery shells fired at school classes, right above her head, ”said Orlando Bloom.

He visited the school, which is only three kilometers from the front line and was damaged by shells.
Currently, UNICEF is helping to repair and rebuild 57 schools in eastern Ukraine, and is also providing hundreds of thousands of children on both contact lines with textbooks and stationery kits. Children are provided with psychological assistance and explanatory work is carried out among them about the danger associated with mines and ammunition.
11-year-old Liana told how she hid in the basement of her school Giovanna Barberis. According to her, since the beginning of the conflict, 55,000 landmines, unexploded ordnance and ammunition have been found and cleared. “And we know this is just the tip of the iceberg. We must ensure that every child has the opportunity to safely get to class, learn and play,” said the UNICEF representative.
UNICEF continues to work in the regions of Ukraine affected by the conflict. In 2016, the Children’s Fund plans to renovate another 60 schools and 80 kindergartens, as well as provide psychosocial support and mine awareness to 400,000 children.

Since then, Dorsey has changed a lot – both externally and internally.
He begins his speech with a description of the Golden Gate Bridge, talking about its impeccable design, engineering merits and unrivaled utility. Some of Square’s former employees are now calling that speech the birth of a new Jack Dorsey, his reincarnation after his humiliating Twitter exit.
The 38-year-old Dorsey could now be in charge of two companies at once. Firstly, he plans to retain the position of CEO at his current company Square, which is preparing for an IPO this year, and secondly, he may be officially appointed CEO of Twitter (appointed October 5, 2015 – ed. note). .) , where he has been temporarily acting as CEO since June of this year. “He is very enthusiastic and has ambitious plans for both companies,” says one of his longtime acquaintances.

Like the founder of Apple, Jack Dorsey was a flamboyant and uncompromising leader, who was squeezed out of the company by his own associates. And after a period of exile and oblivion, he triumphantly returns as the savior of an enterprise that is going through hard times. And even his assertion that every manager should be an editor may have been borrowed from Jobs.

So how different is Dorsey now from before?
First, the awkwardness that marked his brief return to Twitter in 2011 as CEO and head of operations has disappeared. Secondly, he now has behind him a company built from scratch, Square, which is already ready for an IPO. And thirdly, according to a Re/code survey among 20 current and former colleagues, Dorsey has matured noticeably.
Transformation
From the very beginning of his work on Square, Dorsey was determined to smooth out the harsh features of his character. Then he realized that perception decides a lot.
As Nick Bilton writes in his book The Twitter Incubator (Hatching Twitter, 2014), many employees used to be annoyed at how Dorsey could leave work early to make it to yoga or art classes. Often he returned to the office late at night after class to finish the work, but no one saw this. So at Square, Dorsey made it a habit to get to work early and stay late, especially if his employees were late.
“He realized that the impression is often the reality,” says one of his former employees. “Therefore, I did everything to create a new impression of myself.”
He radically changed his image: worn jeans, T-shirts and piercings were replaced by expensive suits from Hermes and Prada. Besides him, no one in Silicon Valley wore suits.
Some things changed gradually. Previously, Dorsey personally controlled each individual project, delving into the smallest details of the work, which could bring employees to exhaustion. Now he has become more reserved.
Although, as he got rid of some shortcomings, new ones began to appear. So, if Dorsey saw good potential in a new employee, he immediately introduced him to his inner circle and loaded the poor fellow with overwhelming responsibility. Of course, such confidence in the choice of employees can bring results and inspire a newcomer to work with full dedication. However, according to many of the sources interviewed, this approach backfired in some cases.
“Jack seemed to be extremely inconsistent in his choice of favorites,” says one of Dorsey’s former employees. – We said: “Today you are in Jack’s favorites. Well, wait until tomorrow and everything will change.”
This often happened. Dorsey made such high demands on some employees, while declaring it publicly that as a result they had to either live up to these expectations or leave. Therefore, at least two employees in less than a year after starting work at Square had to leave of their own accord.
Over time, Dorsey took this experience into account. According to sources, he delegated work with personnel to a team of professional managers, who henceforth stood between him and the state of 1,200 employees. Dorsey himself and representatives from Twitter and Square declined to comment on this information.
In a sense, Dorsey was forced into this position. In 2013, a major scandal erupted around Square. Production director Keith Rabois received allegations of sexual harassment from an employee and was forced to leave the company. Rabois himself categorically denied the accusations, and the leadership supported him in this. However, later he was nevertheless asked to leave, as a shadow had already been cast on the company’s reputation. Rabois was considered the right hand of Dorsey and actually supervised all daily work. Many wondered how Dorsey would cope without him.
However, within a few months, Dorsey did something that even the most devoted fans did not expect from him. He hired a team of real business stars. The first was Francoise Bruger, one of the non-public, but very influential top managers of Google. At Square, she led the sales department, her task was to bring the company’s new products to market to generate additional profit. The second was Gokul Rajaram, who until that time had worked as head of advertising products at Facebook.
A year later, Alyssa Henry of Amazon joined them, to whom Dorsey eventually handed over all of his small business development. And that’s not counting CFO Sarah Fryer, a former analyst at Goldman Sachs, and Jess Dorogusker, head of the hardware department. In the end, under the leadership of Jack Dorsey was perhaps the most decorated team of specialists in all of Silicon Valley.
Once the top management of the company was formed, Dorsey had time to thoroughly work on his own reputation as a great thinker, a strategist who anticipates trends long before they appear.
He also has time to personally take on the development of his favorite projects at Square: the cash transfer service Square Cash and the new version of the flagship card reader for bank cards with electronic chips, which are now being rolled out everywhere.

Over the past few months, this organization has allowed him to devote more time to Twitter, even as Square is actively preparing for a stock offering. As it has now become clear, running the company does not require Dorsey to be permanently in the office.

When it became clear that the reincarnation of Square Wallet called Square Order was also not working, Dorsey and his team closed the project within ten months.
While Dorsey hasn’t completely given up on consumer ambitions, he has acknowledged that Square is a company aimed primarily at small businesses, not the retail market, the sources say.
He still demands a lot from his employees, but not impossible. One of his favorite “why?” often confuses employees who do not have experience with him. He loves the process of arguing and demands an answer to his “why?”, even if he agrees with the interlocutor. One of the employees of Twitter describes Dorsey as an amateur to add fuel to the fire.
Behind all this decoration of expensive suits and fashionable haircuts, the old, stubborn and impulsive Jack Dorsey sometimes peeps through.
In the spring of 2014, the Wall Street Journal published an article examining the financial side of Square’s operations. This gave rise to a wave of negative publications that the top management of the company allegedly spends a lot of money from investors. In one of the publications of the time, the Re / code portal reported that Square was not yet looking for a buyer. And although Apple and Google have already begun to look closely, all negotiations immediately ceased. By August, under the onslaught of negative publications, Dorsey could not stand it and convinced the company to publish a rebuttal article called “Top 10 Square Myths”. However, this article only provoked journalists, and new incriminating materials rained down.
And then Dorsey made a completely unexpected decision. He boarded a plane and left San Francisco to tweet a few hours later that he had arrived in Ferguson, Missouri. Here, not far from his hometown of St. Louis, street riots were in full swing after a white police officer shot dead an unarmed black guy. And now the billionaire Jack Dorsey is already walking in the same column with the protesters, is tweeting what is happening and uploading an operational video to the Web. Some employees were outraged by the way the head of the company left the headquarters at the time of the crisis, but the majority still supported his decision. Supporters of Jack Dorsey at that moment were sure that he could not have done better.
“Many people work for Square because they believe in it and in the company’s mission,” admits one longtime employee. “I wouldn’t say that I came here to work just because I wanted to save the world. But I know many employees who would easily be hired by any other company, but they stay here because their leader was able to go to Ferguson and tweet from the front ranks of the protesters.

Since taking over the company in June, Jack Dorsey has become a full-fledged leader, despite the prefix “acting” to his position. It is impossible to explain exactly why Dorsey’s current return seems final to many, but few doubt it. Insiders say they were convinced by the tone in which he talks about the company and product. According to them, Dorsey often communicates with employees via email, sometimes several times a day. In these letters, he explains what he likes about Twitter and what he would like to change.
He now seems a completely different person than Dorsey, who was already returning to the company as CEO in March 2011. Former colleagues recall that then he seemed to be seeking revenge for his expulsion in 2008. Personal loyalty was of the utmost importance, and employees who were loyal to another co-founder, Evan Williams, were often forced to leave. At meetings, he most often sat silently and indifferently. And if he spoke, he spoke so vaguely and indefinitely that few people understood him. Sometimes he could just stand up and write a few words on the blackboard without explaining anything.
He no longer sits silently in meetings. Current peers say Dorsey delivers direct and constructive opinions and recommendations, proving his reputation as a startup guru. There are still some fears that he will break loose and start kicking people out, but the former aggression seems to be gone. And even Twitter co-founder and good friend Biz Stone agrees that Dorsey has changed in Square over the years.
“My impression is that he went into seclusion for about 40 years and all this time he was engaged in self-improvement,” says Stone. – Sometimes it makes me want to ask him, they say, how did this happen? Where did you get all this – this confidence, calmness, knowledge? He seemed to have become deeper, wiser. Talking to him is like talking to someone much older.”
In addition to new communication skills, the current Jack Dorsey demonstrates a willingness to solve difficult or painful problems. Thus, there has long been a discussion within the company about the possibility of changing the limit of 140 characters for messages on the Twitter network. In the past few months, the discussion has been revived at Dorsey’s initiative. And he even announced recently the launch of a new product that will allow users to write messages longer than the notorious 140 characters.
Such innovations may seem insignificant to the uninitiated, but for the company itself it is a huge shift, because Twitter was founded a year before the first iPhone appeared and has survived to this day, when text messages have become the main method of communication. Clearly, Dorsey is not tied to the company’s heritage, even something as sacred and inviolable as the 140-digit message limit. As many insiders say, only someone with the confidence of a founder could have made such a radical break with the past.
Dorsey became active in other areas as well. Just two months into his new role, he rebuilt the core product team, tasking former Crashlytics CEO Jeff Seibert with identifying shifts responsible for the core product. In the past, Twitter has had a practice of ownership of the project. But now the company employs more than 4 thousand people, and it is not always even clear who is responsible for what. So it seems that the company left a lot to chance. (October 12 it became known that Dorsey decided to carry out massive layoffs in the company – approx. ed.) .
As one insider put it, “Twitter is acutely lacking in decision-making structure and accountability at the middle level.”
In addition, Dorsey brought with him a rush style of work. He gave Seibert just a few weeks to determine which of the ten new ideas to start implementing on Twitter right now and which ones to leave for later. “He likes to set hard limits. He believes that this promotes creative thinking and simplicity of solutions, ”says one of the sources.
Difficulties
On her first comeback in 2011, Dorsey struggled to sit on two chairs. He had to divide his time between work at Twitter and at Square, where the first product had just launched. He also oversaw a major redesign of Twitter known as T1, which added the Discover layout. This project was ultimately not accepted, and his work at Twitter gradually came to a standstill, and at the end of 2012 he decided to leave the company permanently in order to devote himself entirely to Square.
Since then, rates have only gone up. Twitter is in real trouble today. Since its public offering in 2012, the company has gone through several major personnel changes and has seen a stagnant trend in user numbers. Eventually, the value of the company’s shares dropped 40% from the initial offering. That’s why the company’s board of directors made it a condition that whoever took on the task of rescuing her must devote all their time to it, and not work part-time.
Meanwhile, Square is preparing for its IPO, and Jack Dorsey will have to lead a road show for several weeks persuading potential investors to support a company that operates in a highly competitive market where giants like Amazon are its rivals.
The good news for investors is that most of the people we interviewed are confident that Jack Dorsey is the best person to manage these two companies.
He has always been meticulous about his work schedule, and this is something he certainly hasn’t changed. Dorsey’s workday now looks like this: he spends the morning in the Twitter office, and in the afternoon he goes to Square, stopping by a small coffee shop located about halfway between the headquarters of the two companies on the way.
The geographical proximity of the two places of work certainly helps. However, most colleagues are confident that Dorsey would have been able to manage to work on two fronts without that. Twitter has long struggled with user growth, and many believe that this problem can be solved in two ways: the development of the main product and competent PR. In both, Dorsey is an unsurpassed professional.
Former and current employees unanimously admit that no one knows the Twitter product like Dorsey knows it. And recently, he has also learned to present his vision well to both employees and partners. He has shown the ability to build a strong management team that will help him manage day-to-day operations in both companies.
In 2010, Jack Dorsey, speaking to the Square team, said that every leader should be an editor first and foremost. The time has come to prove it by personal example.
