Hi! I am Sonja, the director of Childtime. I began working in child care at the age of 18 and I worked with all ages during my first few years in the field. I have worked for Childtime for 22 years. While at Childtime I was the lead teacher, team lead and then I became the director where I remained for 14 years before I became a specialist for Childtime. I am happy to be back home as the director of Childtime on Floyd Avenue.

I continued my educational journey and advanced into a management position as the team lead, then again as the assistant director.

This publication describes the Medi-Cal Waiver program for HIV/AIDS. The program has case management services. It provides direct assistance to people living with HIV/AIDS. These services are provided in lieu of living in a nursing home or hospital. You can use it to help you stay at home. You can use it to help you get home from the institution. The publication tells you who is eligible. It tells you about the services you can get.

What is Alternative Medi-Cal for People with HIV/AIDS?

The HIV/AIDS Medi-Cal Alternative Program (AIDS Program) provides disease management and care services for patients with HIV/AIDS as an alternative to nursing care and hospitalization. You can use these services if you want to stay at home or leave a health facility.

Who is eligible for the program?

In order to receive services, you must meet the following criteria:

  • have a confirmed diagnosis of HIV or AIDS from your health care provider; and
  • have a health status confirming the need for home care; and
  • be eligible for Medi-Cal services at the time of application and each month thereafter; and
  • Evidence of need for a nursing facility stay and a score of 60 or below on the Cognitive and Functional Assessment Scale.

What types of services are available under this alternative program?

  • Nurse and social worker services
  • Household services such as cooking, housekeeping, grocery shopping, etc.
  • Home care
  • Minor changes at the place of residence, for example, the installation of ramps, handrails, widening doorways, etc.
  • Supply of special medical equipment
  • Nursing – registered nurse/certified nurse
  • Psychotherapy
  • Financial support for the upbringing of children/infants
  • Nutrition Counseling
  • Non-emergency medical transportation services
  • Home delivery of food

More Information

Alternative AIDS Program services are provided free of charge, but cost-sharing health care providers must pay a monthly share to be eligible for services. The maximum amount effective January 1, 2017 is $25,727 per year.

It is NOT POSSIBLE to be enrolled and receive services from any of the following programs at the same time: Medi-Cal hospice program; the state program for the provision of assistance on an individual basis; a targeted support program under the Alternative AIDS Program or another Medi-Cal program to provide health care services at home or in the community.

It is POSSIBLE to be a Medicare Hospice member at the same time.

Alternative AIDS Program agencies can provide information about program eligibility, enrollment, and nurse and social worker eligibility. If you are not eligible for an alternative AIDS program, you can try other programs from the same agency or contact the parent company. A list of agencies for each county is provided below.     Quite often, patients go to the doctor with complaints of transient loss of consciousness, and the definitions are very different: fainting, seizure, blackout, etc.

    Such different definitions are due to the fact that sometimes this condition is difficult to describe, it is very often accompanied by fear and confusion. Fear can be due to both loss of control over oneself and physical injury, a fall during an attack.

    Transient loss of consciousness is a rather diverse condition that includes fainting (syncope), epileptic seizures, a critical decrease in blood glucose levels (hypoglycemia), transient disorders of cerebral circulation. Syncope is understood as a short-term loss of consciousness due to a global decrease in blood supply brain, usually leading to a fall.

    TYPES OF SYNOPSIS AND TREATMENT

    There are three groups of syncope depending on the mechanism of their development:

    • neuro-reflex – in which loss of consciousness occurs in certain situations: during prolonged stay in an upright position, with strong emotions (joy and fear), after urination, after eating, after exercise, shaving and others;
    • cardiac — loss of consciousness is caused by pathology of the heart: arrhythmias, malformations, ischemic disease, damage to the heart muscle;
    • orthostatic – in which the regulation of maintaining arterial tone is disturbed, which leads to a decrease in blood pressure in an upright position.

    Depending on the type of syncope, its effect on the quality and duration of life changes, treatment tactics: caused by the fall;

  • cardiac (cardiac) may be harbingers of life-threatening conditions and sudden death from a heart attack;
  • orthostatic increase the risk of developing serious cardiovascular disease and significantly impair the quality of life.
  • It is worth noting that it is the decrease in blood pressure in an upright position that often causes dizziness and reduced exercise tolerance.
    EXAMINATION AND DIAGNOSIS OF THE CONDITION

    In order to help a patient with transient short-term loss of consciousness, it is necessary to determine whether it is a syncope or another condition (epilepsy, transient cerebrovascular accident or another cause), and then prescribe reasonable treatment.

    Examination of the patient begins with a thorough questioning and clarification of the nature of the episode, the circumstances and time of its development, the precursors of an attack and the state after it. The interview is followed by a physical examination, which necessarily includes the measurement of blood pressure and heart rate in a horizontal and vertical position. The follow-up examination is aimed at identifying or ruling out heart disease as the cause of syncope. It is likely that after the first examination of the patient, the diagnosis will become clear, and other studies will confirm it. In young adults, dysfunction of the autonomic nervous system is the dominant cause of syncope, which is not life-threatening. In patients of older age groups, frequent causes are heart disease, these patients are at risk and require special attention. In the absence of heart disease, special tests can be carried out aimed at provoking fainting and elucidating the specific mechanism of their development, which allows developing tactics for further treatment. Unfortunately, patients suffering from syncope are often deprived of adequate medical care. This is due to the low awareness of doctors about the necessary examination algorithm and a lack of understanding of the role of a general practitioner, internist and cardiologist in the treatment of these patients.

    Syncope specialists say that three conditions are necessary for successful treatment: the right doctor, the right place and the right time.
    FEATURES OF TREATMENT AND METHODS OF PREVENTION

    Treatment begins with explaining to the patient the nature of his illness and teaching him methods of preventing the development of syncope and preventing their development (if possible)

    prolonged stay in an upright position (standing), in stuffy and cramped rooms, or provoked by emotional stress. Predisposing factors for its development are hot weather, insufficient fluid intake, drinking alcohol, overwork, taking drugs that reduce blood pressure.
