Abstract


Objectives:

Respiratory infections (RIs) are an important cause of morbidity and excessive antibiotic prescriptions in children attending day care centers (DCCs). We aimed to assess the effectiveness of an educational and hand hygiene program in DCCs and homes in reducing RI incidence and antibiotic prescriptions in children.


Methods:

A cluster, randomized, controlled, and open study of 911 children aged 0 to 3 years attending 24 DCCs in Almería (Spain) with an 8-month follow-up. Two intervention groups of DCC families performed educational and hand hygiene measures, 1 with soap and water (SWG; n = 274), another with hand sanitizer (HSG; n = 339), and the control group (CG; n = 298) followed usual hand-washing procedures. RI episode rates were compared through multilevel Poisson regression models. The percentage of days missed were compared with Poisson exact tests.


Results:

There were 5211 RI episodes registered. Children in the HSG had less risk of RI episodes (incidence rate ratio [IRR]: 0.77; 95% confidence interval [CI]: 0.68-0.88) and antibiotic prescriptions (IRR: 0.69; 95% CI: 0.57-0.84) compared with the those in the CG. Children in the SWG had a higher risk of RI episodes (IRR: 1.21; 95% CI: 1.06-1.39) and antibiotic prescriptions (IRR: 1.31; 95% CI: 1.08-1.56) than those in the HSG. Pupils missed 5186 DCC days because of RIs, and the percentage of days absent was significantly lower in the HSG compared with the CG (P < .001) and the SWG (P < .001).


Conclusions:

Hand hygiene programs that include hand sanitizer and educational measures for DCC staff, children, and parents, reduce absent days, RIs, and antibiotic prescriptions for these infections in children at DCCs.

In 2014, legislation was passed that changed the number of unannounced visits from two per year to one per year in Child Care Centers, Group Child Care Homes, and Licensed Family Child Care Homes. Who can pick up a child from preschool

An acute problem that has been causing discontent on the part of parents for many years is the question: “ WHO CAN TAKE A CHILD FROM KINDERGARTEN
The answer lies in several legal documents:

– the Constitution of the Russian Federation;

– Family Code of the Russian Federation;

– Civil Code;

– An agreement between the preschool educational institution and parents (legal representatives), signed on the day the child is enrolled in kindergarten.

the caregiver has a relationship with the parents of the child and the minor child and no one else. Representatives of minors by virtue of Art. 64 of the RF IC are parents.

If parents entrust someone with their authority to pick up a child from kindergarten, then they must formalize this properly. But this must be adults.

The educator has the right not to give the child not only to minor brothers and sisters, but also to adult outsiders who do not have the appropriate authority to do so.

Babies cannot be transferred to minors, since at this age full legal capacity has not yet come, they are responsible for themselves only within the framework outlined by the Civil Code of the Russian Federation. Their legal capacity is determined by Chapter 3. Art. 21 of the Civil Code of the Russian Federation. Chapter 2.

Article 60 of the Constitution of the Russian Federation.

In fact, this is a very serious issue, since it can affect two problems:

  1. With the minor child attending kindergarten, who was taken away from kindergarten, an accident may occur (he got hit by a car, disappeared, etc.)
  2. A minor attending a kindergarten may himself cause harm to the property and health of other persons (burned in a house, etc.)

In the first and second cases, they will immediately find out to whom the child was handed over, who accompanied him. Parents can say that they did not ask to pick up the child from the kindergarten at all and, accordingly, the kindergarten employees will be held accountable.

In accordance with the provisions of Art. 1073 of the Civil Code of the Russian Federation Responsibility for harm caused to a minor under the age of 14 is borne by the parents or the institution where he was or should have been. If the baby, who was taken by the older brothers from the kindergarten, starts a fire, and the parents say that he was supposed to be in the kindergarten at that time, and he was taken away without their knowledge, then the entire responsibility lies with the teacher and the educational institution, because the teacher entrusted the child to a minor. But if the minor has of the notarized statement of the mother, responsibility is removed from the educator.

AGREEMENT on education between the Municipal preschool educational institution “Kindergarten of combined type No. 90” Teremok “of Petrozavodsk and parents (legal representatives) of pupils attending a preschool institution.

According to the agreement, the parent is obliged: to transfer and pick up the pupil personally from the educator, without delegating these duties to outsiders, and to persons under the age of 18. In an exceptional case, the trustee specified in the contract has the right to pick up the pupil from the institution, with the obligatory presentation of an identity document of the trustee.

Also According to the Agreement and the Charter of MDOU “Kindergarten No. 90”:

  1. The child passes under the responsibility of the educator only at the moment of transfer from hand to hand by the parents and in the same way returns to the responsibility of the parents back.
  2. If parents bring a child before the official start of the kindergarten and leave him in front of a closed door, they are violating their obligations, since before the arrival of the teacher, the child is not yet under the responsibility of the kindergarten staff. Parents who pick up the baby later than the time established by the contract also violate their obligations to the preschool institution.
  3. At the end of the working day in kindergarten, the teacher has the right to transfer the child only to previously agreed persons.
  4. In the event of a danger threatening the child from the side of the picking up adult (drunk state, manifestation of aggression, etc.), the educator has the right not to give the child away. An exception is situations when a person who wants to pick up a child (for example, a biological parent who does not have parental rights) poses a threat to the teacher – in this case, the kindergarten employee is not obliged to put himself at risk.

If the parents did not pick up the child.

Employees of kindergarten have the right to send the child to the children’s room of the police. Kindergarten workers’ actions to send your child to the police children’s room will be completely legal. And this is called, in the language of the law, – prevention of neglect.

An educational institution is responsible for the life and health of pupils during the educational process. The mode of operation of preschool groups, the duration of stay of pupils in them, as well as the workload of students are determined by the Charter of the institution.

In other words, if the parent does not pick up his child from the preschool institution in a timely manner (at the time established by the Charter or the Agreement), then the institution after the specified time (after the end of work) is not obliged to participate in the upbringing and maintenance of the child, and in fact, no responsibility for the life of the child, his health, place of stay educational institution does not bear. Accordingly, employees of a preschool institution are not required to be with your child, spending their personal time on this, and even more so to take the child to their home, or leave it “on bail” to strangers. This duty and responsibility rests solely with the parents.

A child who is not picked up by the parents at the set time is essentially left unattended by the responsible persons. Such a child meets the criteria for a neglected child, since according to Art. 1 of the Federal Law of June 24, 1999 No. 120 “On the Fundamentals of the System for the Prevention of Neglect and Juvenile Delinquency” , a neglected person is understood to be a minor whose behavior is not controlled due to non-fulfillment or improper fulfillment of duties for his upbringing, education and (or) maintenance by parents (other legal representatives).

Such a neglected child shall be sent to the children’s room of the police. The procedure for delivering a child to the police, as well as notification of such delivery to parents and the procedure for issuing a child by parents is regulated by the “Instructions for organizing the work of departments for juvenile affairs of the internal affairs bodies”, approved by order of the Ministry of Internal Affairs of the Russian Federation dated May 26, 2000 No. 569 , and a number of others internal orders.

If the child is repeatedly delivered to the children’s room of the police, the parents may subsequently have problems with the authorities, and the question arises of non-performance (improper performance) of their duties by the parents.

Parent’s Guide

“Who can pick up a child from kindergarten”

1. Parents should be aware that not only they themselves, but also other people can pick up their child from kindergarten. In this case, you just need to correctly arrange everything. Educators of a preschool institution do not have the right to give the child to anyone without the appropriate permission in their hands.

2. When enrolling a child in a kindergarten, one of the parents concludes an agreement with the management of this institution. In it, he prescribes his own passport data, as well as last names, first names, patronymics, passport numbers of those who can later pick up the baby from the kindergarten. When filling in the appropriate fields, it is very important to enter all the data correctly.

3. The parent who draws up all the necessary documents must indicate in the contract that he agrees that his spouse will pick up the child from the kindergarten. This is very important, since from a legal point of view, the agreement is concluded only between one of the parents and the head of the kindergarten. This rule is especially relevant if the marriage between the mother and father of the baby is not registered.

4. In addition to parents, grandparents, close relatives, as well as complete strangers can pick up a child from kindergarten. For example, parents can assign this responsibility to a nanny. The main thing is that all the data of this person be spelled out in the contract. The authorized person must carry an identity document with him. This is necessary so that the teacher can freely release the baby with him.

5. Persons under the age of majority cannot pick up a child from kindergarten. In many preschool institutions, this rule is periodically violated. At the request of the parents, the educators transfer the child to his minor brothers and sisters. From the point of view of the law, this is completely unacceptable and threatens the teacher with a fine or even dismissal.

6. The caregiver has the right not to give the child to a trusted person if something threatens the baby. For example, if the child’s father comes for the baby in a state of intoxication and shows aggression, the kindergarten employee has the right to leave the baby in the group and call the child’s mother, or call the police until the circumstances are clarified.

7. You need to pick up your child from kindergarten on time. In accordance with current instructions, the caregiver should not wait for the arrival of parents or trusted persons for more than 10 minutes. In this case, he has every right to call the police and transfer the baby to the responsibility of the employees of this department.

Helpful Hint: An identity document should be carried even by the parents of the baby. The guard of the institution or the new caregiver have every right to ask for a passport to make sure that they give the child in safe hands.

At the height of the working day, while the kids had an hour’s sleep, the women decided to take out the garbage. And for this they had to go beyond the fence of the institution, just into the very courtyard where something strange was happening. Near the entrance of the five-story building, a scene was unfolding: a man was walking rapidly, holding tightly by the hand a boy of 6-7 years old, dragging him along.

At first glance, it seemed that the father was leading his son, who was capricious. But! The child flatly refused to go. Near the entrance he stopped and rested, then the “father” began to push the child in the back.

– At that moment we saw the man’s face, – Marina Shatrovskaya tells the KP-Irkutsk correspondent. – And they remembered that they had seen him drunk more than once in this yard. But the child next to him was never noticed.

The women began to watch what would happen next. And their doubts were confirmed. Having smoked a cigarette, the man leaned over to the boy, whispered something to him and dragged him by force to the front door. The child at this moment, in every possible way tried to slow down with his feet. Victoria and Marina imperceptibly approached and then they already heard a child crying and the phrase: “I don’t want, I don’t want to go!”

– Where did you drag the child, let him go, – Marina and Victoria could not stand it anymore and shouted.

– This is my brother’s son, everything is fine, – the stranger quickly got his bearings and answered. Then he continued to hold the boy with one hand, and with the other he tried to open the front door.

– Victoria grabbed the man’s hand, and I started pulling the boy out, – continues Marina Shatrovskaya. – I pull, and he squeezes even tighter! The boy was crying and begging to be saved. To attract attention, we also began to shout loudly. At that moment, the man also grabbed Victoria. And it actually helped: he shifted his attention, and we were able to pull the child out.

That was it. The man, having escaped from the shackles of the teachers, ran into the entrance and closed the door. The boy was so frightened that he ran without looking back. He did not stop even when the women shouted to him: “Wait, we will bring you home.” Victoria and Marina went back to the kindergarten, on the way they were overtaken by the boy’s mother. Her son told her everything.

– She began to thank us! She said that she brought packages home and left her 6-year-old son for exactly two minutes at the entrance with car keys, – says Marina. – During this time, a passer-by managed to drag the child away.

The kidnapper was detained in a few minutes by the police, who were called by the teachers. He calmly sat in his apartment and did not even try to resist. Why he intended to drag the child into the entrance, the investigators are now investigating.

– They say he has mental problems, – says Marina. – At the confrontation with us, he smiled and assured that he only wanted to “feed and warm the child.”

According to Komsomolskaya Pravda, the detainee is now awaiting trial, where a preventive measure will be chosen for him. It is possible that the man will be taken into custody. Well, Marina and Victoria, who have been working as educators for almost 4 years, believe that ordinary attentiveness helped them save the boy.

– We can already see the strange behavior of children with the naked eye, – says Marina. – We have kids in the group 4-5 years old, almost the same age as that boy. We didn’t do anything out of the ordinary.

As Artemy Borisov admits, there he even had to replace educators, moreover, in all groups, from nursery to senior. Now he plunged headlong into the work of the manager, but already in Molodyozhny, where he lives.

– Of course, I don’t know all the nuances yet, but I am ready and will learn, – says A.A. Borisov. – I’m not used to it!

It turns out that in his thirties he continues his education. Now he has switched to the second year of the correspondence department of the Altai Institute of Culture. And behind him – and the Tomsk Polytechnic College, where he received a specialty “Development and operation of oil and gas fields”, and the Tomsk Music College. E.V. Denisov (accordion class) … He thought that he would connect his life with creativity, with music – at the age of five he picked up melodies by ear on an old accordion – and here is a new twist.

However, Artemy did not part with music. She dreams of a good professional button accordion, at least one for the local kindergarten and the House of Culture. He continues to play in order not to lose the skill, “loads” his fingers with complex compositions, but this is in his free time.

And his working day begins early in the morning, along with the opening of the ward institution. You need to delve into everything, check everything, from the watch to the kitchen. Now there is a repair, and the main groups are closed, there is only a duty officer. In general, there are many children, according to the list at the end of the school year, there were 128 preschool children.

– Is it difficult to lead a women’s team? – we asked Artemy Aleksandrovich, having previously found out that there are thirty employees in the Molodyozhnensky kindergarten, 13 of them are teaching staff, mostly of the “weaker sex”.

– Our people are all responsible and efficient, they know their work, the scheme has been debugged for a long time, – A.A. explained. Borisov. – But, of course, there will be innovations to make the system work even better. In the meantime, everyone is helping me to understand, especially the senior teacher Yulia Nikolaevna Kondratieva. With such a team, with such support, I think we will cope with all the tasks.

There are no special kindergartens for children with special needs in Germany – all kindergartens (Kita) are inclusive. The kindergarten in Köpenick “Hand in hand” (“Hand in hand”), which we managed to visit, was opened in 1972. Until 1993, there was a special institution for children with serious disabilities. There are currently 125 children studying at Kita. These are 8 groups of 15 children, each of which has children (from 3 to 5) with disabilities (hearing, vision, autism, children with cerebral palsy, Down syndrome). There are 3 educators per group of children, with Both of them have additional special education to work with special children.

Itegrasions Kita “Hand in hand” is one of the city’s seven internship sites, educators and teachers from other preschool institutions can come here with questions or difficulties and problems that have arisen – they will definitely be helped.

The Berlin educational program is the same for all children, both normal and disabled. For children with disabilities, as well as for all others, a portfolio is compiled, which is maintained throughout the preschool period, while an individual program is drawn up for each child with special needs. An individual educational route is developed by educators, in whose group there is a child with disabilities, together with psychologists, defectologists, and speech therapists. All of the listed specialists are not full-time employees of preschool institutions (in Germany, preschool institutions, healthcare and diagnostics are divorced, but at the same time there is a close relationship between all organizations). Every month, all specialists gather to check what has been done and what has not been done, and also to discuss which areas in the development of the child require the most attention.

Methods

At Kita, “Hand in hand” works according to different methods and programs. For example, they use the TEACCH program (an internationally recognized, comprehensive program for people with autism spectrum disorders and their families) and the Marburg Attention Training Program. The Marburg program includes 15 sessions, it can be 15 weeks or 15 days, during which children learn to listen carefully, ask accurately, help each other, work together and, most importantly, increase self-esteem. Children are given attention exercises in a playful way, then a movement lesson is introduced, sensation games are held, and at the end of the session, an exercise is given – massage and exercises for the development of imagination and fantasy.

Each child is given individual lessons, regardless of whether it is a normal child or with developmental disabilities. It is strange in the Russian ear to hear that a child chooses his own teacher, chooses a topic that interests him. On the chosen topic, the selected teacher deals with the child individually for half an hour. Such classes are very important not only for the child – the teacher during the lesson observes the child, his development. Individual classes are provided for all children and are attended by them only if they wish.

Observations and experiments

But, programs and methods are not the most important thing, the staff of the kindergarten believes. Much more important is the inner attitude of teachers and their willingness to learn through observation of children: what are their needs, what topics and interests, how they develop, what they need to develop in order to fully participate in public life.

They like to experiment here. In many different ways. So, once a week, children can come to the kitchen to prepare food under the supervision and guidance of the chef. They also experiment with a variety of objects, for example, pieces of organza or pasta. Do you know how organza will behave if you first squeeze a piece in your hand, and then gradually open the fist? The process of blooming a flower right before our eyes is magnificent and delights even adults! And what kind of pasta can be passed through a cone with a narrow hole, you know? Everything is aimed at arousing curiosity and interest in children, enriching already known processes and topics with new impressions and knowledge.

Workshops

They are different, but the most amazing – in which they draw with their hands, feet, paint themselves and those who are nearby, walls, floors. Everything is possible here. No restrictions. There are only rules.

Barrier-free environment

It is truly barrier-free because caregivers focus on the needs and resources of each child, which find their place in the design of everyday life, games, project work and spaces. All children learn sign language, because there are children in Hand in hand Kita who cannot speak, have speech disorders or have a small vocabulary.

For 6 years the kindergarten has been cooperating with the nursing home. They learn songs together, make crafts, talk, share life stories, dance together. They prepare thoroughly for the meetings and look forward to them. And, by the way, interesting changes are observed by educators – children who find it difficult to concentrate, who have weakened attention, establish closer contact with older people.

Inclusive learning starts in a fun environment where kids love to do things. The main task is to accompany and support children in their quest for autonomy, independence and self-confidence.

Dear parents! In our kindergarten, it is customary to follow certain rules. These conditions are not difficult to fulfill and will help us to find a common language and prevent conflict situations. We hope for your understanding and attitude for a positive result

1. The working hours of our kindergarten are from 07.30 to 18.00 , 5 days a week, days off Saturday, Sunday. On holidays established by the Government of the Russian Federation, the kindergarten does not work.

8. If your child has known allergic reactions (food, flowers, pollen, hair, medicines, insect bites, etc.) , it is even better to provide a medical certificate from an allergist or pediatrician, which reflects this fact.

9. If during the day the child shows the first signs of illness (fever, vomiting, rashes, diarrhea, etc.), the parents will be immediately notified of this and must, as soon as possible, take the child out of the kindergarten’s medical isolation ward.

10. On the eve of the alleged absence of a child in kindergarten for family reasons (vacation, summer period, spa treatment, etc.), leave an application addressed to the head of the MDOU (form on the website).

11. Do not bring a sick child to kindergarten , by doing so you endanger the health of not only your child (the patient needs bed rest, treatment and a mother nearby), but also the health of all other children. Sick children and children with suspected illness identified during the morning filter are not accepted to the kindergarten. 21. Parents are STRICTLY prohibited from entering the playroom, pantry or toilet without the permission of the teacher.

22. If you find any dissatisfaction or some disagreement with the way the children’s life in the group is organized, you should, FIRST of all, discuss this with the group educators or the head educator. THEN, if this conversation did not resolve the problem, please contact the kindergarten management. Of course, you can contact the manager directly and express your concern. But in the vast majority of cases, “questions” are successfully resolved at the “educator-parent” level.

23. Disputes and conflicts with caregivers and other parents must be resolved ONLY in the absence of children.

26. Ask teachers more often about your child’s successes and failures, and not just about those who offended him and what he ate today.

27. Try to know all the professionals working with your children by name and patronymic in order to contact them with any questions you may have regarding the upbringing and education of your child.

28. Come pick up your child in the evening on time . Remember that when you pick up your child at exactly 18.00 or later, you force the caregivers to work overtime for free. Do not abuse the kindness of the kindergarten workers, they also have children. And your child, if you are late in the evening, after 18 hours, according to the law, is considered “neglected”.

29. Entry of private cars into the territory of the institution is strictly prohibited in order to avoid accidents. It is strictly forbidden to park in front of the gate , as this prevents the delivery of food to the additional building of the kindergarten, as well as the passage of official cars in case of an emergency.

Will a license stop kindergarten teachers who like to loose their hands?

When sending a child to kindergarten in the morning, parents must be sure that he will not only be fed and tidy, but also treated kindly. But there are quite a lot of videos of how educators open their hands when the baby cannot answer. Education officials see the solution to the problem in licensing preschool institutions, primarily private ones. Why the need for licenses is perceived by society so ambiguously, understood inbusiness.

During just one day, several events were held at different venues, where one topic was discussed: kindergarten licensing. The fact is that more than a decade ago, when there were not enough preschool institutions in the country, the government decided to remove compulsory licensing in order to encourage the opening of more private kindergartens. And it turned out to be justified, there actually became more of them – however, the question of quality arose. This issue was the subject of an online briefing organized by the Ministry of Education.

Isn’t the devil so scary?

The issue of licensing is not an easy issue and not one day. To begin with, an appropriate bill is needed, a legal framework is being created, and only after that the mechanism will be implemented, said the chairman of the committee for ensuring the quality of education in science of the Ministry of Education and Science of the Republic of Kazakhstan Gulzat Kobenova.

“Today, when opening preschool organizations, there are no uniform requirements, the specified information in the notification is not checked, and a number of kindergartens do not submit a notification about the beginning of their activities at all,” the committee chairman said.

At the same time, she noted that this process is not new, licensing was a necessary requirement until 2011. Since today 99% of children are enrolled in pre-school education, it’s time to return the licenses. Over the past two years, the number of private kindergartens has tripled.

“After all, the issuance of a license relates to the provision of public services. It is fully automated. In addition, preschool educational organizations do not have a direct connection with the licensing authority,” the speaker emphasized.

In fact, a license is a confirmation of the qualifications of employees. At the same time, she stressed that there are no draconian qualification requirements, everything depends on the situation on the market. But it is necessary to do this, the situation has changed, including in society. Gulzat Kobenova noted that licensing of kindergartens is a world experience.

“Preschool organizations are accredited in countries such as Singapore, Malaysia, Korea,” the speaker informed.

Kindergarten – not overexposure

Among the innovations is voucher funding, which was launched as a pilot project in the capital. This implies that parents themselves can choose a kindergarten for their child, which means competition, and therefore better conditions for children.

As the Chairman of the Board of JSC “Financial Center” of the Ministry of Education Askar Ibraimov noted, the voucher system will be tested in various regions.

“The horizon for the implementation of large-scale voucher financing is not yet known, since its implementation will depend on the results of the approbation. Changes and details will occur during the approbation,” the speaker said.

Today there are 15 kindergartens, about 180 parents participate in this mechanism. The geography is planned to be expanded. And in the issue of introducing the voucher system, a big role is given to parents. They choose a kindergarten and regulate the financial flow.
