Based on facility documentation and the Surveyor’s observations, the roster in the Pre-K classroom did not reflect the number of children present in the activity area. There were 21 children in the classroom and 22 children documented on the roster. In the School age classroom there were 14 children in the classroom and only 13 children on the roster.

Based on the Surveyors’ observations, the health, safety, and welfare of an enrolled child was placed at risk of harm in the Two’s classroom, a child was asleep on their cot with a blanket over their head. Staff were unable to see the child’s face.
