In Space #1a, the Permission to Administer Medication Form for one (1) tube of Boudreauxs Butt Paste with Natural Aloe expired on April 17, 2021. In Space #3a/b, the Permission to Administer Medication Form for one (1) Albuterol Sulfate Inhalation Aerosol Inhaler expired on May 14, 2021.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In Space #3a/b, one (1) packet of Be Smart Get Prepared Instant Hand Sanitizer, one (1) Be Smart Get Prepared Instant Cold Compress, five (5) packets of Be Smart Get Prepared Quick n Clean Antiseptic Hand Wipes, and one (1) packet of Proteck Poison Ivy Oak and Sumac Cleanser Towelette, all with multiple warnings, were in an unlocked First Aid Kit stored in an unlocked cabinet under the right handwashing sink in the bathroom used by children. In Space #4a/b, one (1) Renuzit Hawaiian Oasis Air Freshener Gel with multiple warnings was open and on top of the cabinet to the left of the closet used for cot storage. In Space #5b, three (3) aerosol cans of Scrubbing Bubbles Bathroom Grime Fighter, one (1) bottle of Windex Original, one (1) container of Members Mark Disinfecting Wipes, one (1) aerosol can of Febreze Air Linen and Sky, one (1) aerosol can of Febreze Air Heavy Duty, three (3) aerosol cans of Lysol Disinfectant Spray, and one (1) bottle of Goo Gone Automotive Spray Gel, all with multiple warnings, were in an unlocked cabinet under the handwashing sink to the left of the bathroom used by children.

Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. In Space # 3a/b, three (3) packets of Be Smart Get Prepared Sunscreen Lotion, two (2) packets of Be Smart Get Prepared Antacid Tablets, and one (1) packet of Be Smart Get Prepared Non-Aspirin Acetaminophen Tablets were in an unlocked First Aid Kit stored in an unlocked cabinet under the right handwashing sink in the bathroom used by children.
