Smiling faces and nice words prolong the antidepressant effect of ketamine

Researchers found that positive self-representation training enhanced the antidepressant effect of ketamine in patients with treatment-resistant depression. The training, developed by the authors, involves the use of pleasant words and images of smiling actors, combined with photographs of the subject himself and personal pronouns. The results of a study with 154 participants aged 18 to 60 were published in American Journal of Psychiatry.

Ketamine, developed in the 1960s, can be used not only as an anesthetic, it also has an antidepressant effect. Neuroscientists have shown that this effect is associated with the effect of ketamine on opioid receptors and its effect on neuroplasticity: it helps neurons in the medial prefrontal cortex to grow spines that disappear during prolonged stress.

Intravenous ketamine has a rapid effect on the body within 2 to 24 hours. American researchers have found that it can also reduce suicidal tendencies in patients with clinical depression in a fairly short period of time. However, due to the short-term effect of ketamine, its use in long-term therapy still raises concerns due to possible negative effects from a series of intravenous injections of the drug. nine0005

A team of US researchers led by Rebecca B. Price at the University of Pittsburgh decided to test whether it was possible to benefit from behavioral training for positive self-esteem (positive self-association and self-worth) carried out during the interval when ketamine promotes neurocognitive changes in patients with depression. To do this, they invited 154 adults with documented treatment-resistant (non-responsive) depression. nine0005

As a behavioral training, the researchers used automated self-association training (ASAT), a training aimed at reinforcing a positive sense of self, developed based on the principles of conditioning, when in the process of learning one stimulus becomes associated with another. During the session, which lasted 15–20 minutes, subjects were shown pairs of words and pictures for 250 milliseconds or 12 milliseconds to create and reinforce an implicit association of positive traits (the unconditioned stimulus) and self-oriented information (the conditioned stimulus). The unconditioned stimulus was positive words (cute, attractive, etc.) and photographs of smiling actors, while the authors used pronouns referring to the participant (for example, “I”) and photographs of the participant himself for the conditioned stimulus. nine0005

Participants were divided into three groups: 53 participants from the experimental group received ketamine and ASAT, 51 participants from the first control group received saline and ASAT, and 50 participants from the second control group received ketamine and sham training.

Participants completed the Montgomery-Asberg Depression Rating Scale 1 hour before drug administration, 24 hours after administration, and again at days 5, 12, 21, and 30 after administration. Solutions (0.5 milligrams per kilogram of ketamine or 50 milliliters of saline) were administered to participants via drip over 40 minutes. The day after the injection of the solution, the participants came to the first of eight training sessions. The sessions went on for four days in a row, twice a day. nine0005

The authors used linear regression with mixed effects to analyze the data. After administration, ketamine reduced depressive symptoms over 24 hours (p < 0.0001). Treatment efficacy based on Depression Rating Scale scores for the ketamine groups was 52 percent and remission was 28 percent. For the non-ketamine group, the efficacy was 25 percent and 4 percent, respectively.

Depression scores for the ketamine plus ASAT treatment group were relatively low throughout the month compared to the saline plus ASAT group (p = 0.0004). The scores of the third group, which received ketamine and sham training, increased throughout the period, starting from the second day.
nine0005

Thus, the authors were able to prolong the effect of ketamine with positive self-representation training for at least 30 days in patients with treatment-resistant depression.

Researchers are constantly trying to find new treatments to help patients with resistant depression. So, psilocybin has previously shown its effectiveness in combating various types of depression, and Swedish psychiatrists suggested that patients who do not help antidepressants should consider electroconvulsive therapy. nine0005

“Grandma’s recipes” competition is looking for smiling faces of everyone in the family

13

There are many delicious dishes prepared in different regions of our country. Scientific research and various culinary magazines are devoted to them, covering the delicious map of Bulgaria. The discovery and preservation of authentic recipes passed down from generation to generation in Bulgarian families is the idea of a thematic competition of children’s drawings and essays entitled “Grandmother’s Recipes”. nine0005

Children’s art, created literally in all corners of our country, is collected by the organizers, and after a careful selection, the best drawings are awarded. Each time in the more than 10-year history of the competition, several recipes for traditional dishes typical of this region of Bulgaria are submitted. That is why the new theme of this year’s competition is authentic dishes. Thus, along with a collection of children’s drawings, we have a collection of recipes that are not so popular, but are really valuable for any cook who relies on tradition and authentic taste. nine0005

“However, the goal is not to set records for the number of collected drawings and recipes, but to provoke the creative thinking of children and, above all, to put emphasis on preserved family traditions,” says the initiator of the competition, Penyo Ivanov:

“This year I deliberately focused on works that show the family. Brother, sister, mother, father, grandparents or one of these figures – I pay a lot of attention to the drawings in which these people are depicted, because seeing multimember families is joyful and important. nine0040 The more crowded the drawing, the more joy and mood it brings. Despite the strange times in which we live, the family is alive and traditions and ancient values are important to them. It is even better when we see this living world through the eyes of children, what is there, in the drawings, all together. This is cause for optimism.”

Characteristically, in the Grandma’s Recipes competition there are no restrictions on the technique used by children. They have absolute freedom to choose their means of expression. And this year, “again, there are dozens of recipes written down by the children themselves, which means that there is interest in the topic,” says Penyo Ivanov:

“That’s right, because we are connected with food every day, moreover, most of us spent our holidays with our grandmother in the countryside and kept delicious memories. Childhood memories are something very important, and they remain in the mind for a long time. In general, the competition presents bright drawings that reflect the children’s worldview.”

The artist Maria Petkova, head of the children’s drawing school in Pervomai, also participates as a member of the jury in the “Grandmother’s Recipes” competition. nine0005

“I have a broad view of children’s art, but this is my first time as a jury in a competition. It’s very exciting, and I was under the impression that children, despite the circumstances, need a topic to work with. It is useful for them to be able to express themselves, to show themselves, it is very important for their development, for the formation of self-confidence and striving for success. Children’s creativity inspires us adults, but we must help children. Sometimes they do not know what to draw, and here the role of a parent or teacher is to help. We need to find a way to encourage children to think more, to try different things. Most of them prefer to follow the trodden path – with this competition we aim to encourage those who made their own drawings, but in no case are we against the help of parents.
