Home Care Aide Application Process

Home Care Aide Application Process

To apply online for Home Care Aide registration, please have a credit card ready and follow the steps below:

1. Access the Guardian Applicant Portal at https://guardian.dss.ca.gov/Applicant. Home Care Organizations can submit applications on behalf of Home Care Aides using their Guardian account at https://guardian.dss.ca.gov/agency.

2. Create an Account by clicking “Register as a new user.” A temporary password will be sent to your email account. Your Login username is your email address.

3. Enter Application Information. When prompted for a PIN (Personal Identification Number), enter either the Home Care Organization’s PIN (provided by your employer) or enter R38XKSPE (the PIN for independent home care aides).

You will be required to pay a fee of $35.00 to register; payment can be made by debit or credit card. If applicable, await Home Care Organization confirmation.

4. Retrieve the Live Scan Form. Print the pre-populated form and submit fingerprints with a Live Scan vendor.

To apply via mail for Home Care Aide registration, please have a check or money order ready and complete the following steps:

To renew your registration online, please have a credit card ready and follow the steps below:

1. Access the Guardian Applicant Portal at https://guardian.dss.ca.gov/Applicant. Home Care Organizations can submit renewal applications on behalf of Home Care Aides using their Guardian account at https://guardian.dss.ca.gov/agency.

2. If you have not created an account before, please click “Register as a new user”. Your Login username is your email address. A temporary password was sent to the email account you used to register.

3. Create Application. For “Request Type” select “Home Care Aide Registry Renewal”.

4. When prompted for a PIN, enter either the Home Care Organization PIN (provided by your employer) or enter R38XKSPE (the PIN for independent home care aides).
You will be required to pay a fee of $35.00 to renew your registration; payment can be made by debit or credit card.

Disclosure of HCA Personal Information

For all registrations and renewals, the Department is required to provide the name and telephone numbers of Registered HCAs who do not opt-out of this requirement to labor organizations when the labor organization asks for the information. This means that if you do not opt-out of this requirement, your full name and any telephone numbers on file with the Department may be released to a labor organization.

The law states that the labor organization can only use your information for employee organizing, representation, and assistance activities and it also states that the labor organization cannot disclose your information to any other party. However, the Department has no authority over labor organizations or control over what they do with the information. Additionally, if you opt-in at any point during your registration, there is a chance that your name and phone numbers may be provided to a labor organization and the Department cannot retract any personal information once it has been released to the labor organizations.

    Termination of the COVID-19 Vaccination and
    Masking Requirements

    Effective April 3, 2023, the State Public Health Officer Order, Adult Care Facilities and Direct Care Worker Vaccine Requirement was terminated. As a result, there is no longer a COVID-19 vaccination and booster requirement for Covered Workers, which includes but not limited to Registered Home Care Aides (HCAs) and Home Care Organization (HCO) staff with direct contact with clients or prospective clients at HCOs. Licensees are no longer required to maintain records for COVID-19 vaccination or exemption status of Registered HCAs and HCO staff with direct contact with clients or prospective clients. Registered Independent HCAs are no longer required to maintain records of their COVID-19 vaccination or exemption status.

    The California Department of Social Services continues to encourage all Registered HCAs and HCO staff with direct contact with clients or prospective clients to stay updated with COVID-19 vaccinations and boosters, as this remains the most important strategy to prevent serious illness and death from COVID-19.

    Effective May 11, 2023, the California Department of Public Health (CDPH)  sunset its Guidance for the Use of Face Masks. CDPH provides updated masking recommendations in Get the Most out of Masking. As the risk for transmission increases in the community, wearing a mask will be an important consideration for individuals at a higher risk of getting very sick from COVID-19.     Home and Community-Based Services (HCBS) Spending Plan

    California’s HCBS Spending Plan provides $3 billion in enhanced federal funding to support initiatives that enhance, expand, and strengthen Medi-Cal’s HCBS services. The CMS approval of the state’s proposal will support services provided by six state departments. On March 11, 2021, President Biden signed the American Rescue Plan Act (ARPA). Section 9817 of ARPA provides qualifying states with a temporary 10 percentage point increase to the federal medical assistance percentage (FMAP) for certain Medicaid expenditures for HCBS from April 1, 2021 through March 31, 2022.

    States must use the federal funds attributable to the increased FMAP to supplement, not supplant, existing state funds expended for Medicaid HCBS in effect as of April 1, 2021. In addition, states must use state funds equivalent to the amount of federal funds attributable to the increased FMAP to implement or supplement the implementation of one or more activities to enhance, expand, or strengthen HCBS under the Medicaid program by March 31, 2024.

    As submitted to the Centers for Medicare & Medicaid Services (CMS) on July 12, 2021, and updated on October 27, 2021, California’s initial HCBS Spending Plan and initial HCBS Spending Plan Narrative involve several the state departments, and total approximately $3B in enhanced federal funding for the following categories of services:

    Home Health Assistants care, housekeeping, and other related home health services to help a person continue to live in their home environment in the event of disruption due to illness, disability, social disadvantage, or other problems.

    Personal Care Assistants – New York State Certified PCA is a person who provides nutritional support, personal care assistance, and maintains the environment a person needs to stay in their home.

    Registered Nurses – A New York State (NY) licensed physician responsible for providing direct skilled nursing care to patients. Provides shift and private nursing services. Responsible for planning, coordinating, delivering and evaluating direct medical services to patients and their families in accordance with the professional practice law and agency policy.

    Physiotherapists – Licensed by the State of New York (NY) responsible for providing physical therapy services to individuals who demonstrate functional limitations. The role of the PT is to provide skilled intervention to improve the patient’s level of functionality in terms of self-care, mobility, safety, range of motion or strength, and to develop a safe and effective care program.

    Occupational Therapists – New York State licensed OT who is responsible for helping patients acquire the skills needed to perform ADL. OTs focus their interventions on patients’ upper limbs and on the fine motor skills required to perform functional activities such as eating and dressing. In addition to helping patients develop self-care skills, OTs are involved in assessing the safety of patients’ homes and suggest modifications to improve patients’ ability to function independently.

    Speech Therapists – ST is licensed by the State of New York and must be certified in clinical practice in speech by the American Speech and Hearing Association. ST services are provided to individuals who have speech and language problems, including language difficulties or delays. Speech and language problems can be both expressive and receptive. In addition, ST services are also beneficial for patients suffering from dysphagia. ST works with patients to improve their ability to self-administer ADLs related to their communication or swallowing, such as when talking on the phone, eating, or managing oral secretions.
