Wilder Penfield: neurosurgeon who unraveled the mysteries of the mind to treat epilepsy

127 years ago, on January 26, 1891, American-born Canadian neurosurgeon Wilder Penfield was born. He was engaged, first of all, in epilepsy surgery, but he made a great contribution to the development of medicine and science in general. Today, Google dedicated an image to Penfield on its home page.

Together with the electrophysiologist Herbert Jasper, Penfield developed a technique that allows using electrical stimulation of certain parts of the brain to evaluate the functions of its various structures, as well as to localize the epileptic focus. During the operation, the patients were conscious and described their sensations. This allowed the neurosurgeon to create new functional maps of the cerebral cortex and, for the first time, accurately map the cortical areas related to speech.

In his book The Mystery of Mind, a neurosurgeon tells how he sought to prove that the mind is entirely dependent on the brain, but he came to the conclusion that the picture could be more complex: “Mind always takes precedence over content our consciousness. It is a completely independent entity. The mind commands, the brain obeys. The brain is the messenger to consciousness,” he wrote.

Penfield’s patients included the Soviet physicist Lev Landau, winner of the 1962 Nobel Prize in Physics. In the same year, he was in a car accident, after which, as a result of numerous fractures, hemorrhages and head injuries, he was in a coma for 59 days. After that, he almost did not engage in scientific activity, however, according to his wife and son, Landau gradually returned to his normal state. Unfortunately, in 1968, the physicist passed away for reasons unrelated to the functioning of his brain. While helping Landau while he was in a coma, Penfield found evidence that even if motor control is completely lost and the brain is unable to continuously record streams of consciousness, conscious perception persists.

Some of Penfield’s statements suggest that he was a witty and ironic person. In particular, he owns the phrase “Even the greatest people, unless they die young, sooner or later must bow to pain in the spine.

A day in history: discoverer of oligodendrocytes and homunculus

On today’s doodle, all lovers of neuroscience could see a picture with a brain and a portrait of a bald man. Today marks the birth of Wilder Graves Penfield, who is most often remembered for his discovery of the motor and somatosensory homunculi, the “virtual men” in our brain that illustrate the distribution of areas responsible for body movement and sensory perception. Someone sings praises to him, someone, on the contrary, believes that the concept of homunculi greatly slowed down the development of neuroscience. However, in reality, the scale of this person is much, much more than just mapping two regions of our brain. But first things first.

Our hero was born on January 26, 1891 in Spokane, Washington, 150 kilometers from the US-Canadian border. His father was Charles Samuel Penfield, a highly successful physician.

However, Penfield Jr. studied at Princeton, and if the Princeton-Plainsboro Teaching Hospital really existed, Penfield would probably have had a chance to work in the same hospital as Dr. House. And so, during the training, the young physician was torn between the granite of science and sports – he turned out to be a great American football player. Moreover, immediately after graduation, Penfield was immediately hired by the university – not as a teacher, but as a football coach.

After studying with Sherrington, he worked “at the bottom”, as a nurse in a military hospital near Paris (recall, the First World War was still going on at that time), then he got married and returned to the USA and began to study already at Johns Hopkins University, where he received the prefix MD to his name and surname at 1918 year.

In the spring of 1924, he goes to work for five months with the greatest histologist and fabric dyeer (and also an athlete, artist, and so on and so forth), Santiago Ramón y Cajal. More specifically, having learned various ways to stain nerve cells, Penfield began working with Cajal’s student and collaborator, Pio del Rio Gortega, the man who would later formulate the concept of microglia in detail. By the way, it is important that he does it at 1932 in chapter Microglia , written for the book that later became a landmark – Cytology and Cellular Pathology of the Nervous System . Who do you think will be the editor of this book? That’s right, Wilder Penfield. In the meantime, five months of work will lead to two articles: in 1924, an article by Penfield himself will be published in Brain magazine, in which for the first time in history he confirms the existence of oligodendrocytes, the “third element” of Cajal – non-stellate non-neuronal cells that form the myelin sheath of neurons (the Spaniards unanimously consider the discoverer oligodendrocytes of del Rio Gortego for article 1921 years old).

There are many illustrations on two dozen pages showing this cell type for the first time. And in 1927, Penfield and del Rio-Gortega published a joint article in which they consider how nerve tissue reacts to damage and forms scars.

However, further Penfield still made a choice towards neurosurgery. In the same 1924, Penfield underwent an internship with the remarkable neurosurgeon Otfried Förster, who had just returned from the USSR, where until the last days he was the personal physician of Vladimir Lenin (we have already written more about Förster). It was Förster who “infected” Penfield with the study of epilepsy (at 19In 1930 they will even publish a general article.

However, in order to operate on patients, it was still necessary to obtain an appropriate American education. And here Penfield also learned from the best – the door to American neurosurgery was opened to him by Harvey Cushing, who is considered one of the fathers of modern neurosurgery.

Penfield did his most famous work on the material of operations for epilepsy. By this time (1928) he had moved to Canada, where he received positions at McGill University and the Queen Victoria Hospital, becoming Canada’s first neurosurgeon. At 1934 Penfield received Canadian citizenship.

But back to our homunculi. On December 1, 1937, an article by him and Edwin Baldry was published in Brain magazine, in which, on the material of 163 patients, whom Penfield operated on for epilepsy (under local anesthesia, so that the patients were conscious and could communicate with the doctor). Penfield stimulated various points in the brain with electric current and monitored the reaction. Then Baldry carried out, as they say now, data processing and built the ratio of brain regions and zones that are responsible for the movement and sensitivity of body parts.

At the very end of the article, the famous homunculi were drawn, one of the most successful metaphors in the field of neuroscience: men, whose body parts are proportional to the areas of the cortex, responsible for the sensitivity and movement of these parts. Big face, hands, tongue, small arms and legs… It must be emphasized that Penfield understood well the conventions of this approach, and the fact that brain areas do not have clear boundaries, and that different people can have zone shifts… Now this concept a little outdated, but it’s too early to hand it over to the museum. In the end, it is to her that we owe, for example, the emergence of “brain-computer” interfaces with feedback, when the signal from the sensor on the prosthetic hand goes to the electrode implanted in the “homunculus” and the person feels with mechanical fingers.

It must be said that Penfield’s work was not limited to the somatosensory and motor cortex. His research has made an important contribution to the study of the role of the parietal and temporal lobes of the brain. Electrical stimulation of these zones on the material of 520 patients was able to cause in 40 of them visual, visual, auditory and olfactory hallucinations, as well as the “out of body” syndrome, also first scientifically described by Penfield. He was a scientist and a pioneer in the neuroscientific study of the phenomenon of déjà vu. At the same time, all his life he was engaged in both conventional neurosurgery and neurology, he treated patients: it was Penfield who was invited to the brain-injured Nobel laureate Lev Landau, and his recommendations made it possible to gradually return our physicist to intellectual activity. If he had not died as a result of arterial thromboembolism after surgery, then who knows …

Few people know, but Penfield was twice nominated for the Nobel Prize in Physiology or Medicine in 1953 (maybe he was nominated later, but the Nobel committee database is open only until 1967). Alas, then it did not grow together: that year the prize went to Hans Adolf Krebs, for the discovery of the corresponding cycle. It’s a pity, because there is still no Nobel Prize “for experimental neurology”. Penfield has left his name in science fiction: in Philip Dick’s famous Do Androids Dream of Electric Sheep, characters use the Penfield Mood Organ, a device that can be evoked at will.
