Working as a doctor in Germany
Deficient German profession – a doctor. How to become a doctor in Germany. Recognition of medical education and immigration.
Working conditions of doctors in Germany
German doctors receive high salaries while working on modern equipment in comfortable conditions. Therefore, not only patients, but also doctors aspire to Germany. The path to becoming a doctor is not an easy one. In return, career prospects and a stable, well-paid job are offered.
nine0002 German medicine – leading in the world. The German healthcare system has unlimited possibilities for providing first-class medical services. Compulsory state health insurance for residents of Germany allows you to treat burghers without worrying about the solvency of clients.
High-quality service generates demand among foreigners as well. Patients from different countries tend to make appointments at clinics. Thousands of visas for medical treatment in Germany are issued every year. nine0134 A large number of patients requires a corresponding number of staff.
The availability of medical services is ensured by a dense network of clinics and private offices.
The Germans treat doctors with great respect. According to surveys, physicians annually lead the list of the most respected professions. The second are nurses.
Career opportunities for doctors in Germany
German doctors work in various types of institutions:
- employees hospitals – Krankenhaus
- employees of university clinics who are involved in research in addition to treatment – Universitätsklinikum
- owners of own private practices – Arztpraxis
- employees of medical societies , for hire or as entrepreneurs – Gemeinschaftspraxis
In addition to direct patient care, the doctor also has a chance of finding a job as
- consultant for pharmaceutical companies,
- official in public health authorities.
60% of those with higher medical education work as attending doctors.
Clinics, hospitals, hospitals in Germany receive millions of patients. The waiting list is scheduled for several weeks or months in advance.
But work in clinics involves a heavy load. 60-hour working weeks, daily shifts, participation in scientific research allow you to make good money, but we are not talking about peace. Therefore, doctors who have gained experience use the opportunity to work as private entrepreneurs in the status of a free worker, opening their own medical offices. Here the doctor himself decides how many patients to accept per day and how often to go to work. nine0003
In Germany, the gradual replacement of individual practices with collective practices and clinics continues. There is a gradual rejection of the classical private practice model. The thesis is confirmed by the figures – the owners of practices are getting older: 45% of self-employed doctors are 50-59 years old, 20% – 60-65 years old and 10% over 65. At the same time, 44% of employed doctors are 35-49 years old.
It is possible that young employed doctors will eventually quit and open their own practices, as a praxis job is an interesting option for a later career period. nine0003
In pharmaceuticals, physicians are involved in the production of medicines. Finding new drugs, conducting experiments, doing laboratory work, and obtaining approval for use require excellent education and extensive treatment experience.
Health departments use physicians to organize control and supervision. Prevention of epidemics, verification of hygiene standards, analysis of medical errors – only a small fraction of the possible duties.
Salaries of doctors in Germany
nine0002 The salary of a physician depends on the place of work .
Tariffs
In clinics, salaries are calculated according to the tariff agreement – Tarifvertrag. In addition to the medical title and position, service life in the specialty is also taken into account.
Item | € |
---|---|
Head Physician – Chefarzt | 100000-280000 |
Senior Physician, Head of Department – Oberarzt | 100000-130000 |
Specialist – Facharzt | 65000-85000 |
Junior employee, intern – Assistenzarzt | 48000-55000 |
Approximate general data does not reflect subtleties. There are several tariffs in Germany. There are 3 groups:
- for uniclinics
- public city hospitals
- private hospitals
For example, the starting monthly salary of a junior doctor according to the tariff contract for 2018
- in clinics – 4630€
- in public hospitals – 4400€
You can see the current tariff patches depending on the level and period of work in full here.
Private doctors
It is difficult to estimate the income of independent practitioners. A lot of factors influence the overall profit:
- praxis size
- whether operations are carried out
- Availability of service personnel
- opening and closing times
- doctor’s specialization
According to statistics, after deducting the costs of medical treatment, a private doctor in Germany is left with 200,000€ per year. In general, the total annual income of the practice varies from from a couple of million euros for radiology to 400000 for a pediatrician .
Anesthesiologist salary in Germany – 3472-6779€
Anesthesists are needed in four different areas of medicine: surgery, treatment, emergency, pain management. Any serious clinic needs an anesthesiologist, but many specialists also work for themselves. Their services are used as assistants in case of operations by other medical institutions.
Salary of an ophthalmologist in Germany – 6098-11288€
An ophthalmologist in Germany (Augenarzt) treats patients with eye and vision problems. Tasks include diagnosis, prescribing therapy and medications, and issuing prescriptions for glasses. With the appropriate skills, the doctor is able to perform operations. nine0003
To become an ophthalmologist in Germany, you will have to study for 5 years at a German university, pass a state exam, and then go through a long-term procedure for obtaining the qualification of an ophthalmologist while obtaining a medical specialization – Approbation.
Eye doctors in Germany work both in their own practice and for hire in clinics and hospitals, as well as in emergency centers.
Salary of a surgeon in Germany – 3942-11955€
Surgeons (Chirurg) are one of the most difficult professions in medicine. Six years of study and years of practice pay off handsomely. Few people in Germany earn more than surgeons. nine0003
Most often, doctors operate in clinics, hospitals and hospitals. But there are also private practices in which a company of two or three surgeons usually works for themselves, performing not the most complex operations.
Salary of a dentist in Germany – 2649-5387€
What dentists (Zahnarzt) do is known to everyone who has teeth.
Dental studies in Germany last approximately 11 semesters. Admission to higher education in this specialty has norms for the average score of the certificate. To work as a dentist, a foreign specialist will have to go through a year of practice and pass a state exam. nine0003
Dentists in Germany work mainly as private entrepreneurs. Earnings are generally higher than average, but it strongly depends on the qualifications of the specialist, as clients try not to go to bad doctors.
Shortage of medical workers in Germany
German clinics have been reporting a shortage of specialists for years. If you count the open vacancies, several thousand highly qualified professionals are missing.
In reality, not all positions are posted for recruitment purposes right now. Often HR departments conduct interviews with an eye on the future so that they have a range of candidates on hand in case of a sudden loss of employees. Therefore, there is a real shortage of personnel, but the criticality of the problem is exaggerated. nine0003
Really big difficulties with waiting for a doctor’s appointment in Germany are observed in small remote settlements. Young people tend to work in big cities in fashionable modern clinics stuffed with the latest equipment and using the latest methods of therapy. Nobody wants to treat the sick in the village. Therefore, no one takes over the private praxis of retiring village doctors.
But the shortage of doctors in the German countryside does not help foreigners in terms of finding work. To recognize a foreign diploma in Germany, you need to complete an internship at the Assistenzarzt position – in fact, a position close to an internship. After practice, you will have to pass the state exam. Rural praxis do not usually provide such vacancies. nine0003
At the regional level, Germany is strongly skewed towards the southwest. Here the economic situation is better, salaries are higher, which attracts professionals from all over the country. In the north-east of Germany, gaps are being formed, from where intelligent specialists leave, and not only in the field of healthcare.
But in the country as a whole, the number of doctors is constantly growing, even excluding retired specialists. And the share of foreigners in the overall statistics is increasing every year.
Without foreign doctors, patients in some regions of Germany would be left without service – Economic Week newspaper, March 2019

Doctors’ opinion: there is not enough time in the country for doctors to devote to patients. If urgent action is not taken and more doctors are not trained, the shortage will become more acute. As the population ages, the need for treatment increases. The problem is not in quantity, but in throughput. The number of medical training places should be increased by at least 10%. nine0003
Despite the rapid growth in the number of doctors in Germany, there are still not enough of them.
Health insurance companies see the problem differently: due to the inevitable shortage of doctors outside the major cities, insurance companies believe that the problem is the distribution of services.
We must ensure a better distribution of doctors. But this is impossible as long as there are many places in cities for doctors in joint praxis.
To resolve the issue, it is necessary to reduce the number of practices in places of overabundance. The released doctors should be redirected to regions of Germany where there are not enough doctors. nine0003
A step towards solving the problem may be the obligation of practitioners to work not 20 hours a week, as it was, but 25. This will reduce the waiting time for patients. The issue of material incentives for medical specialists is also being discussed so that the place of residence of the doctor is located close to the practice. Doctors do not agree with the obligation to increase the time of admission – such a coercive measure does not contribute to the desire to open their own office.
The total working time of praxis doctors (not only the appointment time) is still more than 50 hours a week today. The situation is similar in hospitals: 50% work 49-59 hours per week at the rate, 20% – from 60 to 80 hours, including duty and overtime.
Recognition of medical education in Germany
Since April 2012, a new law has come into force, which established the procedure for confirming the profession of foreign doctors. Now foreigners from abroad make a request for the recognition of a diploma in Germany. Conducting medical activities without permission is prohibited.
If the education is equivalent to German, immigrants receive a temporary limited admission and for two years work as an Assistenzarzt – without specialization – with the prospect of confirming a degree and lifting restrictions. nine0003
If the exam is not taken within the allotted time, the permit is withdrawn. Successful completion of the confirmation procedure allows you to continue your career development in the direction of Facharzt through Approbation.
Finding a job in Germany is not easy for a foreigner. If there are problems with German, it is generally unrealistic.
It is also difficult to get a certificate of conformity of the diploma. Especially when you have not completed an internship or similar clinical practice in your country. In many cases, it is necessary to rely on the help of consultants who are familiar with the confirmation processes in the federal states. nine0003
Requirements for a person applying for recognition of the medical profession:
- German at level C1;
- absence of violations of medical ethics and laws;
- the state of health allows you to conduct professional activities.
It will not be possible to learn a language to such a high level in a short time. Start learning German early.
Do not forget that Germany is a federal country. The procedures for recognizing medical degrees vary slightly from state to state. nine0003
Medical training in Germany
Not the last role in the resulting shortage of personnel is played by the local system of medical education.
Germany’s institutions of higher education are sent an order for the number of places for training in the medical professions through the central office in Dortmund. The curriculum approval procedure is the prerogative of the local land authorities. Each land decides who to enroll as a student in medical specialties.
To study medicine at a German university, a foreigner will have to study at the Faculty of Medicine for a year in their home country and enter the Studienkolleg at the university in the direction of M-Kurs. nine0003
The profession of a doctor among Germans is considered one of the most prestigious. Therefore, the requirements for knowledge for admission to medical specialties are the highest. To become a psychologist, for example, you need to study at school only perfectly. With a GPA less than 2.0 – in Russian 4.0 and above – becoming a German doctor is not worth even trying.
The application must be submitted via the websites uni-assist.de. There you can find a database of German universities with addresses, phone numbers, dates of acceptance of documents and other useful information. nine0003
Education in German medical universities lasts at least 6 years and 3 months, including the preparation for the final exams. The last year of study is an internship. Another 3 months is required to work in the nursing service. 2-4 months are devoted to practice in the chosen specialization.
In the first two years, physicians complete the pre-clinical part of the training. At the end, the first exams are taken:
- physics,
- chemistry,
- biology,
- physiology,
- anatomy,
- psychology and sociology.
Then the clinical part begins, the result of which is a year of employment in the clinic.
