SUMMARY:

Provides personalized supports to ensure supported clients’ safety and health is maintained and they can experience the nature and quality of life of their choice based on their needs, preferences, aspirations and decisions. Service provisions focus on maintaining or enhancing the clients’ dignity, well-being, autonomy, personal choice, self-esteem and teaching functional skills that are alternatives to maladaptive behaviors. Supervision, protection and care of clients individually and in-groups at all times. Documentation of their progress; identification of the possible need for professional services; and communication of such findings to Supervisor.

DUTIES AND RESPONSIBILITIES:

May include

but are not limited to the following:

Responsible for the Care, Health, Safety, and Supervision of clients, this is the number one priority

Shifts on floor of 6 to 8 hours daily.

Must have a willingness to move programs as directed by supervisor as needed and/or without notice.

In charge of keeping facility in compliance.

Responsible for maintaining a clean, safe, stimulating living environment. Perform normal household cleaning, including vacuuming, sweeping, mopping, dusting, scrubbing, washing dishes, laundry and keeping closets/drawers organized and neat.

Assist residents with all household chores. Maintain the house neat and clean at all times. Responsible for the overall cleanliness of the home.

Monitor or provide assistance and/or instruction to individual with dressing, grooming, bathing, toileting, eating and sleeping habits as needed.

Assist with wheelchair use, including pushing, folding/unfolding, and loading into autos.

Support Supervisors in creating menus, shopping, and safe food storage for client.

Prepare, provide instruction and/or assist in preparing nutritious meal that reflect individual preferences.

Will follow the direction of the Supervisor to implement the program plan.

Review documentation and discuss items with staff.

Will assist in keeping accurate data on the residents’ progress towards their individual goals.

Engage the residents on all tasks, training and community integration activities.

Report all incidents and occurrences to Supervisor including all incidents of possible abuse or neglect.

Report to Supervisor any resident’s needs that implies to be requested to the agencies.

Will always present yourself appropriately, and as a role model for clients and other Direct Care Staff at home and in the community. Follow dress code.

Will attend any meeting and training per Supervisor request.

Will dispense or supervise dispensing of medication to clients.

Support Supervisors by coordinating staff for medical appointments and activities.

Assist Supervisors in the creation of the Activity Calendar with the staff and clients’ input.

Follow activities scheduled, keep residents active and always give them a choice.

Provide transportation to clients.

Follow established formal behavior support plans and guidelines for positive behavior support programs as outlined.

Assist in developing positive behavior supports by providing feedback about clients’ preferences.

Support client’s participation in cultural, recreational, and educational activities both at home and in the community.

Follow established emergency plan and procedures.

Will document all occurrences and observations on their proper forms every shift.

Provides leadership by displaying positive conduct and demonstrates initiative to enhance program.

Resolves routine problems that arise and contact Supervisor if serious problems arise (behavioral crisis, medical crisis, etc.)

Ensures proper maintenance and care of existing supplies, furnishings and equipment.

Receive receipts and documents for client finances.

Maintains clients’ confidentiality whenever appropriate. Refers sensitive communications to FOS. Maintains Confidentiality Regs.
