Report: Parents Struggle with Cost, Shortage of Early Care and Education Opportunities

L.A. County Child Care Planning Committee Calls for State, Federal Investments to Address Persistent Gaps in Quality and Access to Subsidized Care, and Professionalizing the Early Education Workforce

LOS ANGELES – Parents with two children may pay nearly half their wages for child care in Los Angeles County, even as licensed early care and education centers are only able to serve 1 in 7 working parents with infants and toddlers, according to a new report that explores the resources and gaps in the early care and education system within the county.

The findings are part of The State of Early Care and Education in Los Angeles County: Los Angeles County Child Care Planning Committee 2017 Needs Assessment, which identified a persistent and severe lack of affordable, high quality early care and education for infants, toddlers and school-aged children in the county.

Produced by Los Angeles County Child Care Planning Committee, the Los Angeles County Office for the Advancement of Early Care and Education and First 5 LA, the report prompted calls for significant state and federal investment and policy changes to address the chronic gaps in care parents of young children face every day. Research shows the early years of a child’s development lay the foundation for success and sets them on a path that yields long lasting benefits.

“There is an extreme gap between the number of working families with infants and toddlers and the capacity of licensed early care and education providers to care for those children,” said Michele Sartell, Child Care Planning Coordinator for the County of Los Angeles Office for the Advancement of Early Care and Education. “Our intention with this report is twofold: identify the magnitude of the problems parents face in accessing quality preschool and child care, and identify workable solutions for policy makers at the local, state and federal level to address these repeated gaps in our early care and education system. ”

The countywide assessment, which is conducted every five years, focuses on three essential components of the early care and education system: access to (including affordability of) early care and education; quality in early care and education; and the early care and education workforce.

The report offers several recommendations for policymakers to consider, based on the expertise of Committee members. These recommendations are part of a larger solution that includes increased investment by the state and federal government.

Among the report’s key findings and recommendations:

Child Care is a Costly Expense for Many Families

• A family’s average cost of care in the county is up to $10,303 a year per preschooler in center-based care. Parents with two children earning the county median family income of $54,194 would need to spend nearly half of their income (45 percent) on child care. For families below the poverty line, the situation is even more dire. Even though California minimum wage has increased, income eligibility for subsidized child care has not increased since 2011.

• Recommendation: While subsidized care does bring relief for some families, eligibility requirements are frozen at 2007-2008 State Median income levels. The report recommends updating the eligibility guidelines to reflect the current State Median Income (SMI) and establish up to 12-month income eligibility for families up to 85 percent of the SMI.

“Income eligibility for subsidized care is a barrier for many low-income working parents in need of funded care programs and services,” said Debra Colman, vice chair of the Los Angeles County Child Care Planning Committee and senior program officer at First 5 LA.

There Are Not Enough Services for Toddlers and Infants

• There are 650,000 children under age 5 in Los Angeles County, nearly a third of all children 0-5 in California, yet the current amount of licensed centers and family child care homes have the capacity to serve only 13 percent (or about 1 in 7) working parents with infants and toddlers, and only 8 percent of school age children. Similar shortages exist for families who qualify for subsidized infant and toddler care: only 15 percent of those infants and toddlers who qualify are currently receiving subsidized care in Los Angeles County.
• Recommendation: Increase State and federal investments in child care subsidy programs, especially for infants and toddlers. Advocate for additional funding for subsidized infant/toddler care through increases in State programs like California Center Based Programs (CCTR) for Infants and Toddlers and Alternative Payment, as well as federal initiatives like Early Head Start.

Only 1 in 4 Child Care Sites Participate in a Quality Rating and Improvement System

• Research has found there is a direct relationship between child care quality and child academic achievement and cognitive and language development. The Quality Rating and Improvement System (QRIS) was established to assess the quality of provider services, including child development, social readiness, teaching and environment, and help providers improve the quality of their programs. Yet, in Los Angeles County, only 4 percent of family child care homes and 18 percent of center-based programs participated in the rating program in 2016.
• Recommendation: To ensure the needs of Los Angeles County’s children can be served by high-quality programs, public funding to support local QRIS efforts is needed to reach more providers and help them improve their programs.

Workforce Earns Low Pay, Needs More Education Opportunities

• Teaching infants, toddlers and preschool age children requires the equivalent level of skills and knowledge as teaching older children, yet the pay for the early care and education workforce is less than half of what kindergarten teachers make. Los Angeles County early care and education staff make an average of between $11.73 and $14.75 per hour. While research tells us that children develop better when cared for by higher-skilled workers, only 24 percent of workers in Los Angeles County have an associate’s degree, and 21 percent have a bachelor’s degree.
• Recommendations: Raise regional market rates and standard reimbursement rates for early care and education providers and advocate for state lawmakers to adopt a single statewide reimbursement rate for all providers that covers the true cost of care. For higher skills, expand free and low-cost professional development opportunities and pathways to pursue higher education to improve quality.

The Child Care Planning Committee (Planning Committee) and the Policy Roundtable for Child Care and Development (Roundtable) promote policies designed to increase the availability of and access to affordable, high-quality early care and education programs for all children and their families in Los Angeles County.

• According to U.S. Census 2015 estimates, LA County is home to 10,038,388 people. Of these, 641,635 children are under 5 years of age.
• 18.2% of all people in LA County live under the poverty level, including 26.6% of children. This is higher than the State average1.
• In 2015 the median income in LA County was $56,1961. However, the average family who receives child care vouchers from Alliance agencies earns significantly less. A 2012 study found that the average income of a parent enrolled in one of these child care voucher programs was about $16,241.
• Parents with two children earning the County median family income would need to spend nearly half of their income (45%) on childcare. For families below the poverty line, the situation is even direr.

“Child daycare facility” means a facility that delivers nonmedical care to children below 18 years of age in requirement of personal services, supervision, or service essential for nurturing the activities of daily living or for the safety of the individual on less than a 24-hour basis. Child daycare facility mainly comprises daycare centers, employer-sponsored childcare centers, and family day care homes.

• “Daycare center” means a child daycare facility different from a family daycare home, and includes infant centers, preschools, illustrative daycare facilities, school-age child care centers, and includes child care centers permitted under Section 1596. 951.
• “Employer-sponsored child care center” means any child daycare facility at the employer’s area of business operated directly or through a provider contract by any person or commodity having one or more employees, and available exclusively for the care of children of that employer, and of the officers, managers, and workers of that employer.
• “Family day care home” means a facility that regularly gives care, protection, and supervision for 14 or fewer children, in the provider’s own home, for a duration of fewer than 24 hours per day, while the parents or guardians are absent, and is either a large family daycare home or a small family daycare home.
• “Large family daycare home” means a facility that provides care, safety, and surveillance for 7 to 14 children, inclusive, including children under 10 years of age who live at the home, as outlined in Section 1597.465 and as clarified in regulations.
• “Small family daycare home” means a facility that provides care, safety, and surveillance for eight or fewer children, including children under 10 years of age who live at the home, as outlined in Section 1597. 44 and as clarified in regulations. A small family daycare home or large family daycare home includes a separated single-family residence, a townhouse, a dwelling unit within a residence, or a residential unit within a covered multifamily residence.

A small family daycare home or large family daycare home is where the daycare provider lives and includes a residence or a residential unit that is rented, leased, or owned.

1. “Probation” means the duration that a licensed child day care facility is required to acknowledge with specific terms and conditions set forth by the department to stay or postpone the cancellation of the facility’s license.

Definitions: Adult Day Care vs.

Adult Day Health Care

Adult day care (ADC) provides frail seniors and persons with Alzheimer’s supervision and care in a structured setting during daytime hours. This allows their primary caregivers to work or take a break from their caregiving responsibilities. There are two primary types of adult day care programs. Adult social day care offers basic health services, supervision, meals, socialization, and activities. Adult day health care (ADHC) provides intensive health services for individuals who might otherwise need to live in a skilled nursing community. Some centers offer both types of care. And there are some centers that specialize in Alzheimer’s or dementia care, referred to as Alzheimer’s Day Care. Many centers provide transportation to and from the center at no cost or on a per ride basis.

“Adult day care is the most economical approach to provide supervision and personal care for an elderly loved one on a daily basis.” 

Regulation of Adult Day Care

All adult day care centers are required to have state licenses or are regulated to a degree to provide certain types of services.  This is especially true of those centers that receive public funding, which most do.  Most staff members receive background checks and many are certified social workers. In addition, many states regulate the staff-to-patient ratio and the minimum staff available at any time.

In adult day care, there is on average one care worker for every six participants.  State regulations vary with some states as low as a 1:4 ratio and others as high as 1:10. 

How Adult Day Care Works

Once specific care requirements have been determined, the family will contact a center by phone for an initial conversation about matching needs with their services.  Most families then proceed to visit the center twice, first without the individual in need of care and then again later with their loved one.  At that point, if all parties feel comfortable with the center, it is common to conduct a week-long trial of their services before making any longer term contractual agreement with the center.

Medicare’s Benefits

As most elderly Americans have Medicare, the question if Medicare will pay for adult day care is very common and should be addressed at the forefront of this discussion.   Medicare does not pay for any form of adult day care.  There is some confusion about this question, partly because Medicare is often mixed up with Medicaid. And partly because there are certain Medicare / Medicaid combination programs that cover adult day care. To be clear, a person with only Medicare insurance coverage will not receive adult day care as a benefit.

Medicare’s Advantage

Medicare Advantage (MA) plans, often called Medicare Part C, offer another way for seniors to receive their Medicare benefits. MA plans are available via private insurance companies and often provide additional benefits that are not covered by traditional Medicare. For instance, persons may receive dental and vision coverage, gym membership, and new in 2019, supplemental health care benefits, such as adult day care coverage, to prevent premature nursing home placements. Medicaid

In all states, Medicaid pays for adult day care and / or adult day health care. This is especially true if an individual would otherwise require full-time nursing home care. Each state has several Home and Community Based Services (HCBS) Medicaid Waivers or 1915(c) Waivers that are offered as an alternative to nursing home care. These waivers help individuals remain living in their homes and communities by providing the necessary support services. Very often, this includes adult day care and transportation assistance to and from the community. Unfortunately, waivers are not entitlements and wait lists may exist.

Adult day care is also covered under many Medicaid State Plans (regular Medicaid programs). State Plan Medicaid is an entitlement; so eligible families may use this as a funding source while wait listed for a HCBS Waiver.  Typically, regular Medicaid programs have more restrictive financial criteria than Medicaid Waivers.

Veterans’ Programs

VHA Medical Benefits
The VA will pay for adult day health care but not adult day care.  Adult day health care is included in VHA Medical Benefits Package.  However, one cannot simply enroll in a program. They first must be assessed and found to have a clinical need for the level of care and supervision provided in adult day health care centers.  While the VA will provide assistance, they may not necessarily pay for 100% of the cost.  Depending on the individual’s financial resources and level of need, a co-payment may be required.

Veterans’ Pensions
Specific pensions, such as Aid & Attendance, are intended to be used to provide care for ailing or disabled veterans. Both adult day care and adult day health care fall squarely within that definition and thus can be used in that capacity.ly within that definition and therefore can be used in that capacity.

Veterans Directed Care
As implied by the name, these VA Medical Center based programs give veterans considerable latitude on how they can spend the funds allocated towards their care. Putting funds towards adult day care is well within the regulations. Read more.

Non-Medicaid State Assistance for Adult Day Care

Many states have non-Medicaid assistance programs, sometimes referred to as nursing home diversion programs. These programs are intended to prevent or delay the placement of elderly state residents in nursing homes. To that end, these programs will pay for or help with the cost of daytime supervision in a group environment. This is especially true for individuals with Alzheimer’s or dementia. Our organization’s research found state-funded programs in thirty-nine states that can be used for adult day care. However, a well-researched (but not consumer-friendly and a bit outdated) report produced for the Office of the Assistant Secretary for Planning and Evaluation found just 24 states. It is strongly suggested that families undertaking research on adult day care review both our list below and the Secretary for Planning and Evaluation study.

Tax Deductibility of Adult Day Care

Should a family be unable to find financial assistance for adult day care, it is worth noting the cost of adult day care is almost always tax deductible.   It can be included as a Medical and Dental expense.  Alternatively, if your loved one is your dependent, the costs can be deductible under the Dependent Care Credit.

Costs of Adult Day Care

Adult day care and adult day health care are, without question, the most economical way to provide supervision and personal care for an elderly loved one on a daily basis. When compared with home care for an equal number of hours, adult day care usually costs at least 50% less. Nationwide in 2018, the projected average daily rate for adult day care is $70. It is worth noting that most centers bill in half-day or 4-hour increments.
