Abstract
Background:
Recently, Kingella kingae (K. kingae) has been described as the most common agent of skeletal system infections in children 6 months-2 years of age. More exceptional is the clinical presentation in clusters of invasive K. kingae infections. We describe the investigation of the first outbreak of 3 cases of arthritis caused by K. kingae documented in Spain detected in a daycare center in Roses, Girona.
Patients and methods:
In December of 2015 surveillance throat swabs obtained from all attendees from the same class of the index daycare center were assessed to study the prevalence of K. kingae colonization. The sample was composed of 9 toddlers (range: 16-23 months of age). Investigation was performed by culture and K. kingae-specific RT-PCR. Combined amoxicillin-rifampicin prophylaxis was offered to all attendees who were colonized by K. kingae. Following antimicrobial prophylaxis, a new throat swab was taken to confirm bacterial eradication.
Results:
K. kingae was detected by RT-PCR throat swabs in the 3 index cases and 5 of the 6 daycare attendees. Cultures were negative in all cases. After administration of prophylactic antibiotics, 3 toddlers were still positive for K. kingae-specific RT-PCR.
Conclusions:
Clusters of invasive K. kingae infections can occur in daycare facilities and closed communities. Increased awareness and use of sensitive detection methods are needed to identify and adequately investigate outbreaks of K. kingae disease. In our experience, the administration of prophylactic antibiotics could result in partial eradication of colonization. No further cases of disease were detected after prophylaxis.

I thought: “Is life a comedy or a tragedy?” Somewhat beautifully written, but the thought in a serious way rested here; a comedy or a quiet, eerie tragedy in which all of us, from Napoleon to Bory, are clumsy, stupid actors, especially Napoleon with crossed arms and a cocked hat. She loves her child, respects, is jealous, wants the best for him – a lot of things, but invariably, all her life – she regrets. Here Nature has ordered for us. Take away her pity, leave her higher education, the ability to educate, respect … Leave her everything, but take away her pity, and life in three weeks will turn into a global mess. Why do the people rise all in anger when the enemy is on the threshold? Because everyone feels sorry for all mothers, children, native land. It’s a pity! You may not agree, but I also know about sacred duty, and about honor, and dignity, and so on. But still – in a huge measure – it’s a pity.
Well, it’s time to go back to Bora. I didn’t watch him on purpose, but I thought about him a lot. All day long in the ward, in the corridor, one could only hear:
— Beer-et! And mom? .. Pidet?
– Borya will come, she will come, where will she go. Let me get some rest from you.
Borya laughs, happy that his mother will come.
— Bus, bus?.. Yes?

Even when we go to bed, Borya keeps asking:

Once I carried out, as I now understand, also a rather clumsy experiment. Borya was sitting on a bench in the yard… I sat down next to him and called:
— Borya.
Borya turned to me, and I began to look attentively into his eyes. I stared for a long time … I wanted to understand: is there even a spark of reason there, or has it died out a long time ago, completely? Borya also looked at me. And I didn’t stumble – as happens with sane people – on any thought that I would read in his eyes, on any silent question, on any bewilderment, to which we, looking into the eyes of the sane, immediately also silently answer – bewilderment, contempt, defiant: “Well?” In Borya’s eyes, there is a comprehensive, calm benevolence, such as wise old people have. I became uncomfortable.
“Mom is kicking,” I said, and I felt completely ashamed. And getting up and leaving right away is also a shame.
– Mom pee? Yes? Borya laughed, happy.
— Mom is crying, crying, — I looked around — is anyone watching me? It would be terrible. For everyone, it’s somehow easy, it turns out like that. “Mom is crying, Borya! Pidet. And that’s it. And they go about their business – smoking, washing, drinking medicine. I was sitting on a bench, as if rooted to it, did not dare to say again: “Mom pidet.” And I couldn’t leave either – it seemed to me that I would hear the most insulting, most destructive thing that a person has in store, laughter in my back.
– Bus? Yes?
“Yes, yes, he will come by bus,” I said, and looked away.
– Pivet! Borya exclaimed and shook my hand. Even die, it seemed to me that he was mocking me. I got up and went into the room. And then he imperceptibly watched Borea – if he was laughing, looking at me from his bed. You have to be careful with these people.
Borya can sit motionless on a bench for a long time… He sits, looks thoughtfully in front of him. At such moments I look at him from the side and think hard: does he really know how to get angry? He arranged a scandal at home because they didn’t buy a rose for him. He burst into tears, began to throw chairs, his mother turned up, his mother pushed, his father … Then why is he a half-wit? This is a perfectly reasonable solution to the issue: to take out annoyance on loved ones, we all do it. Or does he not understand what he did? A lunatic with long arms, narrow-minded showed up in the next ward. I’m afraid of officials, salesmen, and people like this gorilla. And they somehow guess who is afraid of them. Once, one official condescendingly, smiling a little sadly, told me for two hours how a kulak bullet hit him here, in the neck … “It’s good that it ricocheted, otherwise it would . .. … “And I strained with all my might, in every possible way showed that I believe him, that I was very interested in all this.
As soon as he came, the gorilla lassoed me in the corridor and told me for a long, stormy time how he had hit his mother-in-law, neighbor, wife… That he had a police passport. “I’m a kid with a cheerful soul, I don’t like it when they start me…”
Somehow a gorilla came into our ward, laughing.
— This, your fool… gave him a cigarette: eat, I say, sweet. Ate all!
We were silent. When a boor appears like this, a big boor, and says with a laugh that he just did something nasty, then everyone becomes bitter. And they are silent. They are silent because it is useless to talk. Here you must immediately beat a stool on the head – the only way to tell the boor that he did not do well. But fuss here, in the ward, no one is going to start with him. He would gladly do it. One premature old man, an informer by the inclination of his soul, went to his sister and said that “a boy with a cheerful soul” forced Borya to eat a cigarette. The sister found the “boy” and began to scold. “Kid” cursed her obscenities. Sister to the doctor. Doctor’s order: write out for violation of the regime.
The “kid” left the hospital when everyone was in the yard.
