Abstract

Background:

Recently, Kingella kingae (K. kingae) has been described as the most common agent of skeletal system infections in children 6 months-2 years of age. More exceptional is the clinical presentation in clusters of invasive K. kingae infections. We describe the investigation of the first outbreak of 3 cases of arthritis caused by K. kingae documented in Spain detected in a daycare center in Roses, Girona.

Patients and methods:

In December of 2015 surveillance throat swabs obtained from all attendees from the same class of the index daycare center were assessed to study the prevalence of K. kingae colonization. The sample was composed of 9 toddlers (range: 16-23 months of age). Investigation was performed by culture and K. kingae-specific RT-PCR. Combined amoxicillin-rifampicin prophylaxis was offered to all attendees who were colonized by K. kingae. Following antimicrobial prophylaxis, a new throat swab was taken to confirm bacterial eradication.

Results:

K. kingae was detected by RT-PCR throat swabs in the 3 index cases and 5 of the 6 daycare attendees. Cultures were negative in all cases. After administration of prophylactic antibiotics, 3 toddlers were still positive for K. kingae-specific RT-PCR.

Conclusions:

Clusters of invasive K. kingae infections can occur in daycare facilities and closed communities. Increased awareness and use of sensitive detection methods are needed to identify and adequately investigate outbreaks of K. kingae disease. In our experience, the administration of prophylactic antibiotics could result in partial eradication of colonization. No further cases of disease were detected after prophylaxis.

Borya | Library SERANN

A newcomer was brought into the ward. A hefty guy, full, even with a belly, handsome, about twenty-seven, but with the mind of a two-year-old child. He immediately smiled from the doorway and said loudly to everyone:

— Pivet, Pivet!

Many of those who lay here for the first time knew this guy. Borya. He lives near the bazaar with his father and mother, on Sundays, when there are a lot of people in the bazaar, he opens the window and barks at people, does not bark evilly – it’s fun. He is generally kind.

– Pivet, Borya, Pivet! Why are you here? What did you do again?

The nanny, making a bed for Borya, says:

— Our Borya dispersed his mother and father.

— What are you, Borya?! Did you beat your mother and father?

Borya closes his eyes and shakes his head vigorously:

— Bose no boo, no boo, no boo!.. — there will be no more.

— Why is he them?

– They didn’t buy a rose! He began to ask his mother – buy him a rose, and that’s it.

– Bose no boo, no boo!

– Lie down now and lie down. “Don’t boo!”

— Does mom pee? Borya gets scared when the nanny leaves.

“Mom peeps, peeps,” the patients soothe him. – I dispersed it myself, and now – mom.

The ward became a little livelier. With fools, I noticed, it is much easier, more interesting than with some smart girl who doesn’t get out of his head that he is a smart girl. And one more thing: the fools, as far as I have seen them, are almost always kind people, and it is a pity for them, and inevitably they are drawn to philosophize. In order not to philosophize at the end – this is always bad – I will now say with what approximately thoughts I ended my observations of Borea (I feel like saying it anyway). I thought: “Is life a comedy or a tragedy?” Somewhat beautifully written, but the thought in a serious way rested here; a comedy or a quiet, eerie tragedy in which all of us, from Napoleon to Bory, are clumsy, stupid actors, especially Napoleon with crossed arms and a cocked hat. In vain, after all, they exclaimed: “You must not feel sorry for a person! ..” This, too, is from an awkward, very proud posture. Respect – yes. Only after all, respect is a thing that comes with culture. Pity is higher than us, wiser than our libraries… Mother is the most respected thing in life, the most dear – everything consists of pity. She loves her child, respects, is jealous, wants the best for him – a lot of things, but invariably, all her life – she regrets. Here Nature has ordered for us. Take away her pity, leave her higher education, the ability to educate, respect … Leave her everything, but take away her pity, and life in three weeks will turn into a global mess. Why do the people rise all in anger when the enemy is on the threshold? Because everyone feels sorry for all mothers, children, native land. It’s a pity! You may not agree, but I also know about sacred duty, and about honor, and dignity, and so on. But still – in a huge measure – it’s a pity.

Well, it’s time to go back to Bora. I didn’t watch him on purpose, but I thought about him a lot. All day long in the ward, in the corridor, one could only hear:

— Beer-et! And mom? .. Pidet?

– Borya will come, she will come, where will she go. Let me get some rest from you.

Borya laughs, happy that his mother will come.

— Bus, bus?.. Yes?

— By bus, yes.

Even when we go to bed, Borya keeps asking:

— Is your mother sick?

He doesn’t bother anyone. He endures injections steadfastly, only he squints his eyes tightly and says in amazement:

— It hurts!

And then, with delight, he tells everyone that it hurt.

They don’t laugh at him, they willingly answer that his mother “will come, she will come” more, harder Borya doesn’t know how to ask questions.

Once I carried out, as I now understand, also a rather clumsy experiment. Borya was sitting on a bench in the yard… I sat down next to him and called:

— Borya.

Borya turned to me, and I began to look attentively into his eyes. I stared for a long time … I wanted to understand: is there even a spark of reason there, or has it died out a long time ago, completely? Borya also looked at me. And I didn’t stumble – as happens with sane people – on any thought that I would read in his eyes, on any silent question, on any bewilderment, to which we, looking into the eyes of the sane, immediately also silently answer – bewilderment, contempt, defiant: “Well?” In Borya’s eyes, there is a comprehensive, calm benevolence, such as wise old people have. I became uncomfortable.

“Mom is kicking,” I said, and I felt completely ashamed. And getting up and leaving right away is also a shame.

– Mom pee? Yes? Borya laughed, happy.

— Mom is crying, crying, — I looked around — is anyone watching me? It would be terrible. For everyone, it’s somehow easy, it turns out like that. “Mom is crying, Borya! Pidet. And that’s it. And they go about their business – smoking, washing, drinking medicine. I was sitting on a bench, as if rooted to it, did not dare to say again: “Mom pidet.” And I couldn’t leave either – it seemed to me that I would hear the most insulting, most destructive thing that a person has in store, laughter in my back.

– Bus? Yes?

“Yes, yes, he will come by bus,” I said, and looked away.

– Pivet! Borya exclaimed and shook my hand. Even die, it seemed to me that he was mocking me. I got up and went into the room. And then he imperceptibly watched Borea – if he was laughing, looking at me from his bed. You have to be careful with these people.

Borya can sit motionless on a bench for a long time… He sits, looks thoughtfully in front of him. At such moments I look at him from the side and think hard: does he really know how to get angry? He arranged a scandal at home because they didn’t buy a rose for him. He burst into tears, began to throw chairs, his mother turned up, his mother pushed, his father … Then why is he a half-wit? This is a perfectly reasonable solution to the issue: to take out annoyance on loved ones, we all do it. Or does he not understand what he did? He feels annoyance, but he doesn’t know how to cause offense properly …

A lunatic with long arms, narrow-minded showed up in the next ward. I’m afraid of officials, salesmen, and people like this gorilla. And they somehow guess who is afraid of them. Once, one official condescendingly, smiling a little sadly, told me for two hours how a kulak bullet hit him here, in the neck … “It’s good that it ricocheted, otherwise it would . .. … “And I strained with all my might, in every possible way showed that I believe him, that I was very interested in all this.

As soon as he came, the gorilla lassoed me in the corridor and told me for a long, stormy time how he had hit his mother-in-law, neighbor, wife… That he had a police passport. “I’m a kid with a cheerful soul, I don’t like it when they start me…”

Somehow a gorilla came into our ward, laughing.

— This, your fool… gave him a cigarette: eat, I say, sweet. Ate all!

We were silent. When a boor appears like this, a big boor, and says with a laugh that he just did something nasty, then everyone becomes bitter. And they are silent. They are silent because it is useless to talk. Here you must immediately beat a stool on the head – the only way to tell the boor that he did not do well. But fuss here, in the ward, no one is going to start with him. He would gladly do it. One premature old man, an informer by the inclination of his soul, went to his sister and said that “a boy with a cheerful soul” forced Borya to eat a cigarette. The sister found the “boy” and began to scold. “Kid” cursed her obscenities. Sister to the doctor. Doctor’s order: write out for violation of the regime.

The “kid” left the hospital when everyone was in the yard.

– Goodbye, soap factory urks! he said goodbye loudly. And laughed. I don’t know, I won’t say, but, in my opinion, our most distant ancestors laughed a lot.

Our hospital is outside the city, two kilometers to get to the bus. Four, who are lighter on the leg and more reliable in the shoulders, got up and went to cut across the “kid with a cheerful soul.”

Twenty minutes later they returned, slightly torn, but satisfied. One of them must have closed his left eye for a long time.

The four shared their impressions in a low voice.

— Healthy!..

— Oral?

– Cursing. He began to swing the rights, tore his shirt on himself, proving that he was a thug.

Borya appears on the porch and again rushes to someone with outstretched hand.

– Beer-em!

– Pivet, Borya, Pivet.

— Does mom pee?

– Pidet, pidet.

Hot. If only there was a little breeze, if only somehow this stuffy peace would be stirred up … If only from here – somewhere!

Sergei Makhotin. BORYA LAPTEV

C ergey M Akhotin

Borya Laptev

N Old ladies were sitting on the bench. A boy walked past with a briefcase. The old women followed how he poked
finger in the combination lock, entered the entrance and quietly closed the door behind him.
“I haven’t seen him before,” one said worriedly.
– No, here, – objected another. – From the fifteenth, perhaps, apartments?
– From the tenth, – corrected the third. – Shoemaker’s son.
The old women calmed down and continued talking about health.
The boy’s name was Borya. His last name was not Sapozhnikov, but Laptev. And he lived not in the fifteenth, not in the tenth, but in the fourth apartment. But
He was…how can I say? He was kind of obscure. Even the old women, who knew everything about everyone and received pride from the residents
nicknamed “people’s avengers”, – and they did not show any interest in Bora. What to say about classmates! Especially about
classmates!..
By the way, Borya’s homeliness had its advantages. He was not called to the board for months. Teachers remembered him only when
it was time to display the assessment for half a year. And, ashamed of their own forgetfulness, they gave him four.
Sometimes in the lesson Boris raised his hand, wanting to answer the question correctly. And then a mathematician or an Englishwoman looked at him with
surprised: “New kid, probably. What is his name? ..” The teacher looked at the other raised hand and with relief
listened to the response of a recognizable student.
Borya had no favorite subjects. Once, in some detective story, he read that people with an inconspicuous appearance are taken as scouts.
How they take it there and what knowledge is required for this, he did not know.
In the family, detectives were the only reading. Once a week, my father bought a new book and read it first. Then my mother read. Then
the turn came to Bori. The author of a detective story about inconspicuous intelligence officers had an inconspicuous last name: Johnson or Jackson. There was no way
don’t remember. In the end, Borya decided that he needed to learn English – the main spy language. Soon, however, he became interested
geography.
Everyone brought contour maps to the control. It was necessary to designate the seas washing the northern shores of the country. Borya started with the Barents.
Then he proceeded to Karskoy, which indeed slightly resembles a crow’s wing. The Laptev Sea stretches in the neighborhood. Borya postponed
pencil and thought for a long time.
In the evening he asked:
– Do we have relatives who are famous?
“Your uncle is already famous, so famous,” sighed my mother, looking reproachfully at her father.
“I don’t want to hear about him,” he snapped, not looking up from the detective.
Does he live in the north? Borya asked.
– How do you know that? Mom was surprised.
– By the sea?
– Hold on. – Mom pulled out a chest of drawers, took out a cardboard box and, rummaging through it, took out a New Year’s card with dancing
around the Christmas tree with hares. – That’s what he writes. – Mom put on glasses and read, addressing mainly not to Bora, but to her father: – “I settled down
in a new place. The whole sea is covered in ice. Northern lights, seals. Almost will, and the soul rests. I don’t think about the future, but the road to the past
no. Maybe I’ll stay here forever, by my sea. Do not remember dashingly. Matvey.” – Mom took off her glasses and said sympathetically: – You
would answer him. A native brother.
The father was silent.
Borya smiled and asked his mother nothing more. The information he received was enough.
At the next geography lesson, he extended his hand with such zeal that the geographer Yuryich finally invited him to the blackboard. More precisely, to the closing
the entire map board. Borya grabbed the pointer and proudly began to talk about the Laptev Sea. How huge it is – seven hundred square
kilometers! And deep! He showed the rivers flowing into the sea – Khatanga, Yana and Lena, of course. He circled Taimyr, Severnaya Zemlya with a pointer,
Novosibirsk Islands. He told about the seal and the bearded seal. He was even out of breath, afraid that he would be killed or stopped. But everyone listened
with curiosity.
– Wonderful! – Yuryich praised and reached for Borya’s diary. – Well, how – Laptev! Who, if not you, knows the Laptev Sea.
Five, sit down.
“By the way, my uncle is still alive,” Borya said and went to his desk.
– Well, well, – the geographer shrugged his shoulders, somewhat puzzled by this message. – And who is he?
Borya turned around and said with meaning:
– Matvey Laptev. It is to him that the sea owes its name.
It sounded so unexpected that no one in the class even laughed. Yuryich, considering Borya’s words not a very successful joke, waved
hand and called another student. The lesson continued.
The teacher stayed in the office longer than usual that day. Leaving the school in the evening, he headed for the trolleybus stop
short way – through the nearest yard. Teenagers gathered on the playground, about seven people. Some even smoked. But this did not prompt
geographer to stop, and two words pronounced loudly and cheerfully: the Laptev Sea. Hoping that at dusk they would not notice him, Yuryich
came closer.
– … He did not even immediately guess that the sea was under him, – said the youngest in the company. – Polar night, you can’t see ahead
nothing. Only the blizzard got stronger. Then the hummocks began to come across.
– What are the hummocks? someone asked.
– Well, heaps of ice floes. Then he sees: something is smoking two steps away. And this is a polynya! Wide, do not jump over. ok, on time
noticed. And suddenly – how the water splashes! Uncle Matvey was covered from head to toe. And the walrus emerges. Fangs – from such! ..
“Yes, this is Laptev,” the geographer finally found out. “I gave him a five today. Well, the kid is lying!”
Yuryich was a young teacher. He wanted to see a personality in each student and influence its development. So he went back to school, found
in the classroom magazine, the Laptevs’ phone number and without delay called directly from the teacher’s room.
The father picked up the phone.
– Ivan Stepanovich? It’s Borin’s geography teacher that worries you. Tell me, does Boris have an uncle?
– What is it?
– It seems to me that for Borya he is a great authority. But the influence he has on the boy…
“He can’t exert any influence,” interrupted Borin’s father. – He is a thief and far from here.
– As you said? – the teacher was confused.
– I cleaned the dacha of the regional prosecutor. Received in full. He spent five years in the camp. Now in the settlement. Did Borka do something?
– No, no, – the teacher hastened to assure, for some reason feeling guilty. – Borya is a good student. Got five today.
– Well then, okay, – Borin’s father finished the conversation.
For another five minutes, the geographer Yuryich listened to short beeps, until he finally realized to hang up the phone.
The next geography lesson for the sixth graders was in a week. And all this time, Boris Laptev and his criminal uncle did not go out at
Yurich from the head. It would seem, what is easier – to call Laptev for a conversation, to explain that a lie does not lead to anything good, that
there are many role models around … Here Yuryich frowned, dissatisfied with himself: “I argue like an old grumbler. I lose my pedagogical
flair. Lidia Yakovlevna must be consulted.”
Lidia Yakovlevna worked at the school longer than other teachers. Everyone loved her, even the students. When the geographer looked in after school
literature classroom, Lidia Yakovlevna checked the compositions of sixth graders. Her eyes glowed.
– Just listen, Vladimir Yurievich, what one boy wrote. Topics of the essay “My most vivid impression of summer.” – She is
opened one of the notebooks and read: – “In the summer I visited Uncle Matvey. He lives in the north. One night I woke up from a lingering
howl. Howled a huge wolf. But I wasn’t afraid. Because the wolf was not evil. His uncle tamed him, and he serves him like a dog. I went out
from home, stroked the wolf, and the beast calmed down. Then the northern lights began. The wolf and I admired him for a long time. describe it
impossible. Uncle Matvey said about the northern lights that this sky plays on a multi-colored harmonica.”
Lidia Yakovlevna put down her notebook and repeated:
– “The sky plays on a multi-colored harmonica.” How good! Lots of mistakes, unfortunately.
– Didn’t Laptev write that? Yurich asked cautiously.
– Yes, Borya Laptev. How did you guess? And, you know, I never paid any attention to him. A talented boy, it turns out.
“He lied everything,” said the geographer. “I got it into my head that his uncle is a great man. What to do with it, I have no idea.
Lidia Yakovlevna thought for a moment. Then she smiled again and said:
– These are all childish fantasies. It will pass with time. Do not judge him strictly, Vladimir Yurievich. Who knows, maybe he himself someday
become famous.
The geographer just shrugged and sighed.
Week is gone. At the appointed hour, Yuryich entered the classroom and announced:
I won’t ask anyone today. The sixth graders cheered happily. – Today I will tell you about two brothers, about two
wonderful pioneers. One was named Dmitry, the other Khariton. Their merits to the Fatherland are so great that in their honor
called the whole sea!
Yuryich solemnly looked at the students, looking for Borya Laptev with his eyes. And suddenly he caught himself on the fact that he did not remember his physiognomy at all.
But it was impossible to delay the pause. And the teacher continued the lesson, for which he prepared especially carefully. He was even a little worried.
His excitement involuntarily passed on to his students. Everyone fell silent, listening to the story about the first graduates of the Naval Academy, who passed
exam to Peter the Great himself. And so began – also Great – the Northern Expedition.
– It was a time of unprecedented cold weather, – Yuryich said. – In Holland, the canals froze, in England, the Thames stood up. Came like this
called the Little Ice Age. Boat “Irkutsk” stuck in the mouth of the Kolyma. And then the detachment of Dmitry Laptev moved to dog sleds
and continued his journey to the east, to the shores of the Pacific Ocean, as it was then called …
Fifteen minutes later the class got tired. The adventure is over. The names of the Yakut and Chukchi rivers, capes, prisons mixed up in
heads. “Nothing,” the geographer reassured himself, “they will grow up and understand that the daily painstaking work of creating a new map can also
be a feat. “But in the depths of his soul he regretted that there were no mortal fights with a polar bear in the fate of his cousins, nor
meetings with militant “non-peaceful Chukchi” or shipwrecks. “Don’t lie, really!” he thought with irritation, and again
looked around for Borya.
The bell rang.
– And Dmitry Yakovlevich, the only one of the officers of the Great Northern Expedition, rose to the rank of admiral, – finished Yuryich
lesson. And he asked with a smile: “Does Laptev want to add anything to this?”
“But he didn’t seem to be there today,” someone answered. – Maybe sick?
The sixth graders rushed to the exit. The class is empty. Yuryich stood thoughtfully by the map. Then he exclaimed: “Well, wait for me”! and
hurriedly left the office.
Ten minutes later he stood at the right entrance and looked at the buttons of the combination lock.
– Who are you going to? – asked one of the old women on the bench.
– I need Boris Laptev from the fourth apartment, – Yuryich turned around. – I’m his school teacher.
– Did what? – inquired another.
– Listen, what’s your number here? I don’t have time at all.
– Is there a document? – squinted the third.
But then the door opened and Boris Laptev came out to meet the teacher. With a bruise under his eye. Oddly enough, the bruise really revived him.
a nondescript face, and Yuryich recognized him at once.
– Vladimir Yurievich? Borya muttered in surprise. – You to us?
– Why don’t you go to school? What’s up with your face?
– This is me… This is me…
“Let’s take a walk,” Yuryich ordered in a strict teacher’s tone.
Laptev sighed heavily and trudged after the teacher, dragging the old suitcase, which the angry Yuryich did not immediately notice.
They moved away from the old women and stopped.
– Where are you going?
– I’ll go to my uncle, – Borya answered quietly. Do you remember what I was talking about? The sea was also named after him.
– How so – will you go? – Yuryich was taken aback. – The middle of the quarter! Who will let you go!
– No, I’m not going to study. As soon as I get there, I will go to the local school. There are schools everywhere.
– What about parents? Listen, your uncle is no pioneer. What a fantasy you have driven into your head!
– Do not believe … – Borya sighed. – And you take it and believe.
– Yes, how can I believe it! – Yuryich slapped his palms on his thighs. – And, most importantly, why?
– It’s more interesting that way.
– More interesting? But imagine! Someone will find it uninteresting that twice two makes four. And what about redoing the math? to someone
I want to simplify the periodic table. Someone to rewrite history. Although she already, poor thing, got it …
– Ouch! Borya suddenly cried out, looking frightened behind Yuryich’s back. – Hide, please!
He quickly took a thick notebook out of his pocket and handed it to the teacher. He turned around. Laptev senior approached them with a sweeping step.
Without saying a word, he slapped his son on the back of the head, cast an evil glance at Yuryich, took the suitcase away from his son and shook his head to the side.
at home. The whimpering Borya trotted to the entrance. The father followed him.
Yuryich looked after them and did not know what to do. Catch up with your father and convince him that assault is not the best method of education? But what
offer in return? How to dissuade a student of his folly?
Having decided nothing, Yuryich went home. He was extremely dissatisfied with himself.
In the subway, sitting in an empty seat, he opened Borin’s notebook.
“The ice floe had been drifting for the fourth day already. When a ship appeared on the horizon, Matvey had no strength left to call for help. Fortunately,
he was seen by the vigilant captain…”

Further, it was told how the traveler was picked up by a ship, in the hold of which the criminal boatswain hid smuggled furs. How
Matvey Laptev guessed this, and the boatswain planned the murder, but attacked the wrong one. Because when he attacked, he got such a plop that
flew overboard and ended up in the arms of a polar bear.
– Cool! The young man next to him chuckled. – It’s a pity to go out. If I were a publisher, I would immediately print it. He shifted his gaze to
Yurich and before getting up respectfully added: – Creative success to you!
– Yes, this is not mine! .. – Yuryich shouted after him and blushed.
He suddenly remembered a recent television series that had caused a lot of noise. The author of the script based on his own novel was a well-known and even respected
writer. But the history of the country in its most tragic decades was more like a vaudeville than a drama. And why is Boris Laptev worse! ..
In the evening he got a call from an old friend:
– Listen, drop your losers! I spoke with the professor, he remembers you. Postgraduate studies at the department are provided for you. Don’t miss out
chance, or you’ll be stuck in that stupid school until retirement.
A week later, Yuryich filed a letter of resignation.
An elderly teacher with a lulling voice began to teach geography to sixth graders.
– It’s a pity that Yuryich left, – Sonya Kozodoeva somehow sighed. – The teacher was nice.
– Yes, who needs this geography! Yurka Burlakin grimaced. – For me, for example, it certainly will never come in handy. Yes, and you too.
But the lessons were interesting. He always talked so enthusiastically!..
– Borka Laptev told even more interesting things! – Yurka waved it off.
However, by that time Borya was no longer their classmate. They say he made another attempt to get to the Laptev Sea. And
unsuccessful again. The father, blaming the former environment of his son for everything, either transferred him to another school; Or the whole family moved somewhere.
In a word, Borin’s further fate is unknown to us.
His notebook remained unclaimed. In the end, he ended up at the bottom of a large cardboard box, where Yuryich put old
letters and abstracts. There, in all likelihood, he is still gathering dust.
