A violation was found for which there is no item number. In Space 4 an employee purse was stored in an unlocked storage closet below five feet making the purse and its contents accessible to the children in care. This is a violation of Sanitation Rule 15A NCAC 18A .2820 STORAGE (f) Employee purses and other personal effects shall be kept out of reach of children.

A safe indoor and outdoor environment was not provided for the children. Mold was observed on the carpet in the far-right corner underneath the open ceiling in Space 3.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. A container of Zeps Disinfectant wipes was stored in an unlocked closet in Space 3.

The indoor and/or outdoor premises was not checked once a day, prior to initial use, ensuring debris, and broken equipment was removed and disposed of. One swing is broken and hanging on the outdoor play space for school-aged children and a cracked plastic tub used to store clothing was observed in space 5.

Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. In Space 1 and Space 2 (designated for children ages 0-1 year) plastic zip lock bags and trash bags were stored in the unlocked drawers underneath the changing tables.

Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. One staff person hired on 6/10/22 received a medical report on 6/20/22.

Staff required to receive on-going training had not completed the required number of hours according to their education and experience. Seven (7) individuals have not completed the required number of training hours according to their education and experience.
