Eligibility Forms for Childcare Assistance

Families can apply for child care assistance or to re-certify their child care assistance eligibility using the forms below.

If a family is not currently eligible for child care subsidy, they should fill out the Child Care Assistance Application (CFWB-012) and any additional forms needed based on their reason for care and how they will verify income. Families should collect all required documentation as outlined below in Documentation Required for Child Care Eligibility (CFWB-022). For help completing the application, families can contact their program, email [email protected], or call 311.

Special Referrals

There are a few instances where a different City agency determines a family’s eligibility for child care assistance and the above forms do not apply:

  • Families applying for or receiving cash assistance must apply for child care through their Human Resources Administration (HRA) Job Center.  
  • Families with an active Child Welfare case, preventive and/or protective, must contact their case worker to make a Child Care subsidy referral. 
  • Employed foster parents must contact their case worker to make a Child Care subsidy referral to be eligible for child care assistance.

CCS Redetermination Forms

Until you receive notification by mail from Child Care Services, do not complete the Redetermination form.

If you have already been receiving CCS and your care will soon be ending, you may be eligible to be redetermined for continued care.  CCS will send you notification via United States Postal Service and will inform you that you must submit a Redetermination form and applicable documentation.  Please be aware of the due date given to you for submission of all documents.

The notification you receive will tell you which of the following Redetermination forms is appropriate for you.

Application for employment (sample, form)

Application for employment – whether it is necessary to write it in order to conclude an employment contract with you. Is it necessary to obtain a job application from an employee in order to employ him or her? Whether the state labor inspectorate or the prosecutor’s office checks the availability of this document.

You can also download a job application form or template here. The information in the article is relevant for 2012, 2013 and subsequent years, since there are no plans to change the legislation on this issue. As soon as there are changes, readers of the job application article will be among the first to know about it.

Not required to apply for a job – this is the main point of this article. This means that the employee is not required to write a job application in order to be employed, and the employer does not need to have a job application in the HR records.

The list of documents to be presented at the conclusion of an employment contract is provided for by Article 65 of the Labor Code of the Russian Federation.

Documents presented by an employee to an employer upon conclusion of an employment contract include the following:

  • Passport or other identification document.
  • Employment book, except for the case of the conclusion of an employment contract for the first time or part-time employment.
  • State pension insurance certificate.
  • Documents of military registration for persons liable for military service and persons subject to conscription for military service.
  • Documents on education, qualifications or special knowledge in employment for a job requiring special knowledge or special training.
  • Certificate of the presence (absence) of a criminal record, the fact of criminal prosecution or the termination of criminal prosecution on rehabilitating grounds when applying for a job to which the relevant categories of persons are not allowed.

The above list of documents may be expanded by federal laws, decrees of the President of the Russian Federation, and decrees of the Government of the Russian Federation.

As you can see, article 65 of the Labor Code of the Russian Federation does not mention such a document as an application for employment.

In other regulatory legal acts, there are cases when an employee’s application is mandatory. For example, paragraph 1 of part 2 of Article 26 of the Federal Law of July 27, 2004 No. 79-FZ “On the State Civil Service of the Russian Federation”, a citizen entering the civil service, when concluding a service contract, submits to the representative of the employer, including an application with a request for admission to the civil service and to the position of the civil service. As a general rule, a job application is not required.

On the contrary, part 3 of article 65 of the Labor Code of the Russian Federation prohibits requiring from a person applying for a job any documents other than those provided for by the Labor Code of the Russian Federation, other federal laws, decrees of the President and resolutions of the Government.

Although a job application is not needed, it is very often “required”. For what? – I don’t understand. In this case, below is a sample (form) application for employment.

The habit of writing an application for employment has been preserved since Soviet times. Modern legislation allows you to do without it. However, many employers still prefer to receive an application from a new employee. Why it is convenient and how to apply in different cases, read the article.

Why is it convenient to fill out an application for employment

The list of documents that a potential employee is required to provide you is listed in Art. 65 of the Labor Code of the Russian Federation. First of all, these are:

  • passport;
  • work book;
  • SNILS.

In some cases, additional documents are required. For example, those liable for military service provide a military ID or registration certificate. If you need special knowledge or qualifications for work, you will need a diploma, certificates. And you cannot work with minors without a certificate of no criminal record. There are no applications in this list.

But almost all personnel officers, when hiring, still ask the employee to write the usual application. After all, its main advantage is that it reflects a person’s desire to get exactly this job. The application does not require any strict rules for registration, while it allows you to make marks on the form. Usually, the manager puts a resolution on it: he expresses his consent to accept the employee and makes recommendations for the HR department.

How to fill out a job application correctly

Since the code does not define the concept of a job application, there are no requirements for its preparation. As there are no nuances when writing an application in 2023. It is important that the text contains all the necessary information for concluding an employment contract.

The application is written in free form by hand or printed on a printer.

  1. In the upper right corner, the employee writes the name of the potential employer, position, last name and initials of the manager in the dative case. Example: “To the Director of Vympel LLC Soloviev A.B.”
  2. Under the details of the employer are the data of the employee, his last name, first name and patronymic in the genitive case. Example: “from Ivanov Ivan Ivanovich”.
  3. The word “Statement” is written in the central block, and then the text.
  4. In the final block, the employee puts the date, signature, last name, first name and patronymic.

The following must be visible in the text of the application:

  • the position for which the person is applying;
  • subdivision in which work is expected;
  • start date;
