to enable him/her/ them to work.
Who Is Eligible?

Child Care Funding is available for certain eligible people,
including:
People leaving Family Assistance as a result of employment (Transitional
Child Care).
Employed individuals not receiving Family Assistance, but qualifying
for aid on the basis of income (Low-Income Child Care).
Parents who have a temporary incapacity and who are expected to
return to work within a short period of time (usually 30 days or
less).
Parents required to be absent from the home due to an emergency
situation.

What to Expect
After you receive your application packet you will need to complete
the forms included with it. The application for Child Care is a
shortened application and should be fairly easy to complete.
You may also need to provide ID; birth certificates; Social Security
cards; divorce, separation, or custody papers; and verification
of all income.
Once we get your returned application packet we will review it
for completeness. If we need more information we will send you
a letter telling you what we still need. After all the necessary
paperwork is received we will make an eligibility decision within
30 days from your application date.
Parent Fees
Most individuals pay a portion of the child care
expense to the child care provider. This payment is called the
Parent Fee or Family Share, and will vary according to the family’s income.
Resource Limit
There is a resource limit of $1 Million Dollars for child care assistance.

DAY Center CARE
The Day Center Manager coordinates care in the Day Center. Each month, the Recreation Therapy team develops a calendar offering a diverse variety of programs and activities with everything from dancing, music, and exercise class, to cooking, games, sewing, knitting, even computer classes, bingo, cards, and more. They also plan outside trips to shopping centers, restaurants, and special events like the New York State Fair, Lights on the Lake, and local sports outings (SU Sports and Syracuse Mets).
These activities and outings are designed to help participants learn new activities and do what they enjoy to the best of their abilities, while keeping any activity limitations or barriers in mind. Our goal is to help our participants maintain their individual purpose and enjoy the best quality of life possible.
Personal care is also available, and the Day Center offers a walk-in shower as well as a whirlpool bathtub, for those who have special personal care needs. Home Health Aides are available to assist with these activities. And there’s even a hairdresser on site (for an additional fee).
Our Registered Dietitian coordinates nutrition care by recommending appropriate therapeutic diets and nutritional supplements. The dietitians monitor weight, consumption of meals, and adjust menus in the day center to meet participants’ individual needs. Nutrition education is provided on an individual basis. PACE pays close attention to the individual nutritional needs of the participants we serve.

The Social Workers help coordinate the psycho-social needs with the participants, their families, and the PACE team. They are instrumental in helping address participant or family concerns as they arise. And, social work and dietician services may extend beyond the Day Center into the participant’s home or skilled nursing facility as needed.
PACE CNY has a very active Participant Council and Participant Advisory Committee, which makes recommendations for new programs and offers feedback to the PACE CNY Program so we can continually improve the satisfaction of our participants, as well as the quality of the programs we offer. PACE CNY works to provide a variety of programs that promote socialization, and make every day meaningful for participants.

Dzorak Care Center in Yerevan served as a shelter for 400 children at 1940s. In 1959 it became a boarding school for underprivileged children. Today, Dzorak serves as an orphanage and nursing home for adults with severe physical and psychological disabilities that have outgrown the traditional orphanage.
